A Research On Doctor-patient Integrity Crisis And Counter-measures In China

Doctor-patient integrity is a kind of integrity form in the field of healthcare which based on the common purpose of against disease and suffering with its own ethical connotations. It includes two aspects: the doctor's professional integrity and the patient treatmental integrity. The basic content of doctor-patient integrity is a kind of uneqal responsibility, the doctor's responsibility is in the first and needn't any reflection;the compensation of lack-integrity is inequality, the compensation to the patients is much larger than that to doctor for the lack of integrity. Doctor-patient integrity has its own ethical principles, which including the principle of informed consent, principle of respect and the principle of autonomy. The relationship doctor-patient integrity between those three principles complements each other, the principle of autonomy is the basis, the principle of respect prerequisite and the principle of informed consent methods. Under the demands of justice, doctor-patient integrity is an essence of a self-discipline, expressing the situation of asymmetric information, demanding equitable distribution of medical resources internally and reflecting the respectation to life itself.Because of asymmetric information between doctor and patient,each of them treating the disease as a way of achieving their own unreasonable interests, doctor-patient integrity crisis is a kind of social phenomenon in the medical - health fields, forming a series of disputes and resulting into a vicious circle. It is essentially a lack of moral self-discipline, which means that moral self-discipline is exclused by moral heteronomy. Thus crisis is actually a crisis of confidence, not only the patients has no-confidence to the doctor, but also includes the doctor put no-confidence on patients ,but in the present situation in China, the former is the crux of the problem.Monetization of characteriristical performance,diversifization of responsibility main-body and universazation of areas form doctor-patient crisis of integrity. Its essence is the deprivation of the informed-consent right of patient by medical service providers, the lose of medical justice, the health system itself implication of the crisis and the irrational expansion.of patient's autonomy rights.Economic structure determining the existence state of doctor-patient integrity, doctor-patient integrity crisis of our country arises in a deep economic reasons. Excessive market model does not meet the basic law of medical-health fields' development; medical service providers run after profit seriously under medical and health resources shortage situation; the polarization of the national income distribution of medical services reduces patients purchasing power and so on.these factors form the fundamental economic reasons of doctor-patient integrity crisis. China's medical reform system was designed based on purchasing power,the purchase of medical services that the principle supremacy of economic interests was stressed and that cause health rights of the patients and half or completely elusive.China's reform of the medical system design is to return to health of their own laws, but for the purpose of justice is only to lose the basis of justice, so that the integrity crisis of the system itself get into the deep plight. Such a system of economic interests ahead of public health requirements of the above led to the chase utilitarian efficiency of the medical cover internal demand justice, not the justice of the defence of reasonable, honest system reliability tension weakening.Doctor-patient credibility crisis in the context of this era arises inevitably, the nature of doctor-patient credibility crisis is implicit in the integrity of the plight of actual performance in the medical system itself.The responsibility of patients for the crisis is not because of their low purchasing power, but because of the integrity vision alienation which arises from the desert of asymmetric information. In this situation, asymmetric information leads their rights to non-rational expansion, the patients rights' misunderstanding arising from asymmetry of information, in turn, forces doctor forming the fear feeling, those behaviors of defensive and refusing medical care were appropriately manifested. As the expecting vision of integrity of patients existing,they estabilish formation of inertia of thinking the physician-patient relationship. Patients neglect reflecting their thought methods, which lead them enjoy the result of that and never reflecting it. To reflect their way of thinking is the only way to shrug off the integrity advantage which results expectation dissimilation.In the so-called post-modern society, it seems that rationality has faced an unprecedented setback, but due to the philosophy charm of rationality, it still has tremendous bounding role to the current social problems. The moral conscious of doctor's obligations embodies the actor's sense of moral and ethical behavior from t moral acts from junior to senior, from perceptual to rational course, it is the supreme goal of medical values of doctor.This course is an inevitable reconstruction process to undergo of restructing doctor-patient integrity. And it is also the moral expectations to doctor integrity. The moral conscious of doctor's obligations is a spirit strength from their own moral convictions, demonstrated by playing multi-dimensional function in reconstructing doctor-patient integrity.
