Background and Subject: Transitional cell carcinoma (TCC) of bladder is the mostcommon malignancy of genitourinary system, with the characteristics of highrecurrence rate. Early diagnose and monitering recurrence plays the key role of goodprognosis. There are always some chromosomal aberrations in TCC cells. Multicolourfluorescence in situ hybridization (M-FISH) was a new technology of detectingchromosomal aberrations. The present study aims to evaluate the possibility andeffectivity of M-FISH in detecting TCC of bladder.Materials and Methods: Fluorescent labeled probes to chromosomes 3, 7, 8, 11, 16,17, Y centromeres and 9p21 region were used to assess chromosomal abnormalities inTCC of the bladder. A total of 57 cancer tissues were analyzed. Positive results weredefined according to UroVysion criteria: four or more cells with gains of two or morechromosomes, or more than 12 cells without singal of 9p21. Positive criteria was alsodefined by that of Bubendorf et al., i.e. two or more cells with gain of one or morechromosomes, or with loss of at least one 9p21 singal. On the basis of the results fromcancer tissues, further analysis was performed to 35 urine specimens from patientswith TCC.Results: The rates of chromosome aberrations were 60.4% (Y),59.6% (9p21),56.1%(17), 50.9% (7), 49.1% (8), 47.4% (16), 47.4% (3) and 38.6% (11) in cancer tissues.The sensitivity was 54.4% as using the combination of 3, 7, 9p21 and 17 probe by theUroVysion criteria. The rates of chromosome aberrations in urine sanmples were65.7% (9p21), 61.5% (Y), 60.0% (17), 54.3% (16), 60.0% (8), 48.6% (3), 45.7 % (7)and 42.9% (11). Among the mixtures of four probes, the combination of 7, 8,9p21plus 16; 3, 7, 9p21 plus 16; 3, 8, 9p21 plus 16 and 7, 9p21, 16 plus 17 had the highestsensitivity (65.7%); the sensitivity of 3, 7, 9p21 plus 17 was 60.0%. If using the criteria of Bubendorf et al., the highest sensitivity of 4-probe mixture was 92.3% (8,9p21, 17 plus Y, for male) and 91.4% (7,8,9p21 plus 16), respectively.Conclusions: The M-FISH was an effective method for detecting chromosomalaberrations in TCC. There existed frequent aberrations in tumorous tissues andexfoliated cells of bladder TCC. The high aberration rates in urine samples occurredat 9p21 and chromoaomes Y, 8, 17, 16, 3, 7 and 11. The mixture of chromosomal andband probes may be used for detecting bladder TCC. The data showed that the bestcombination of four probes were 8, 9p21, 17 plus Y (for male) and 7, 8, 9p21 plus 16.
