OBJECTIVE: To assess the value of dobutamine stress 99mTc -MIBI perfusion tomography (DST) for the detection of myocardial ischemia in patients with suspected CAD or previous acute or old MI.METHODS: One hundred fifty one patients (suspected CAD 79, AMI 64, OMI 8) underwent dobutamine stress and resting tomography. Ninety four of 151 patients had coronary angiography(CAG)within 15 days of DST. Significant CAD was defined as more than 50% narrowing in one major coronary artery or its principal branch.RESULTS:I. Detecting myocardial ischemia in patients with suspected CAD Forty eight of 79 patients with suspected CAD had CA6, including 20 with angiographically CAD, twenty four with normal or non- significant stenosed coronary arteries, and 4 with coronary ectasia. Of 31 patients without CA6, four were diagnosed as CAD clinically, and 27 were not.1. Sensitivity and specificity for the detection of CAD. Seventeen of 20 patients with angiographically CAD demonstrated perfusion abnormalities by DST, twenty of 24 patients with normal or non-significant stenosed coronary arteries had normal myocardial perfusion.
