Background: COPD is currently the sixth leading cause of death worldwide and is projected to rank fifth in 2020 as a worldwide burden of disease according to a study published by the World Bank/World Health Organization. COPD is a disease state characterized by airflow limitation that is not fully reversible, with patients showing a progressively declining, largely irreversible, change in lung function. This is associated with chronic airways inflammation and structural remodeling, including loss of alveolar walls, and goblet cell metaplasia with mucus hypersecretion. Inflammatory cells may contribute to the airway remodeling via secretion of proteases, fibrotic growth factors, and cytokines. In turn, airway remodeling may contribute to the clinical symptoms of COPD. Pharmacologic therapy is used to prevent and control symptoms, reduce the frequency and severity of exacerbations, improve health status. None of the existing westen medications for COPD has been shown to modify the long-term decline in lung function that is the hallmark of this disease. The commonly used glucocorticosteroids are expression of pro-inflammatory cytokines or chemokines. However, several long-term and short-term studies showed that glucocorticosteroids are rather ineffective in improving lung function and reducing the airway inflammation in patients with COPD. For these reasons, the urgent question affronting the medical science field is to seek effctive medications for COPD.Objective: Accoding to TCM theory, the position of COPD is in the lung and related with Five Zang Organs, And Deficiency of qi together with blood and phlegm stasis are the key pathogenesis. In the thesis, we observed the changes of ECM and expression of cytokines, as well as observed the effects of YIQIHUOXUEHUATANFANG, professor Wuweiping's proved recipe, which effects are strenthening the function of lung and the kidney, eliminating phlegm and blood stasis and activating circulation in the collateral on COPD rats model induced by cigarette inhalation and intratracheal LPS Exposure.Method: Observed different degree of collagen deposition in small airways on pathological sections stained by HE and Masson, meassure pulmonary function. Assayed HA and LN in BALF by radio-immunity to observe the effects of YQHXHTF on them, and discussed the action mechanism of YQHXHTF on ECM. Observed the expression of TGF- B1 mRNA and MMP-1 mRNA in lung tissue by in situ hybridization and image analysis. Observed the changes of expression of TN- a protein in lung tissues by immunohistochemistry and image analysis.Result: After YQHXHTF treatment, Pulmonary function of COPD rats were improved. Image analytical results showed that after treament, areas of fibrosis were smaller.The content of HA and LN in BALF increased significantly in COPD model group compared with normal group. After YQHXHTF and roxithromycin treatment in the early stage(treatment begin from the 20th day), the content of HA and LN decreased significantlythan COPD model group. After YQHXHTF treatment in the middle stage( treatment begin from the 30th day ) and in the late stage(treatment begin from the 40th day), the content of HA decreased but not so significantly.TGF-B1 mRNA and MMP-1 mRNA expressed in COPD rats lung tissue, there were significant difference between model group and normal group. Image analysis showed the expression of TGF- P i mRNA and MMP-1 mRNA was significantly increased compared with normal group. After YQHXHTF and roxithromythin treatment in the early stage, the expression of TGF- P i mRNA and MMP-1 mRNA weakened compared with model group. There was no significant difference after the YQHXHTF and roxithromythin treatment in the middle and late stage.The expression of TNF- a protein was significantly increased in COPD rats lung tissue compared with normal group. After YQHXHTF and roxithromythin treatment in the early stage(treatment begin from the 20th day),the intensity of TNF- a protein expression was significantly weakened.
