Objective:The aim of this project is to investigate the effects of Fuxin Yixian Decoction (FYD) on left ventricular remodeling (LVR) myocardial fibrosis (MF) after acute myocardial infarction (AMI), and to research the possible mechanism of it. Methods:In this project, the patients were divided randomly into two groups:the control group received normal therapy and the experimental group received normal therapy with FYD. Rats were divided into6groups:group â…  (sham), group â…¡ (MI), group â…¢ (Captopril), group â…£ (low dosage of FYD), group V (moderate dosage of FYD) and group â…¥ (high dosage of FYD). The myocardial cells were cultured in vitro by serum containing FYD and/or Captopril. Before this period, a model which Ang â…¡ induced RCFibs was established. This treatment on patients has been done for4weeks, while this treatment on rats lasted4weeks too. After the treatment, we checked patients and rats Ang â…¡, Aid, Pâ…¢NP, patients'LVRI, clinical efficacy evaluation of TCM and SAQ after treatment, adding rats'LVMI, myocardial CVF. RCFibs was cultured for24h by the serum which contained treatment. The aim has been to observe the inhibition of RCFibs. Results:(1)Plasma Ang II, Ald, PIIINP decreased significantly more after4weeks treatment in FYD group than in the control group (P<0.05). LVRI, clinical efficacy evaluation of TCM, part dimension of SAQ and the total score of SAQ increased more in FYD group than in the control group (P<0.05).(2) Compared with the group â…¡, Three FYD groups of rats decreased significantly more in myocardial Ang!, myocardial Ald, plasma PIIINP, LVMI and myocardial CVF than group â…¡ (P<0.05or P<0.01). Group VI decreased significantly more in Ald, Ang â…¡, Pâ…¢NP, LVMI than group â…¢ (P<0.05).(3)The cell culture shows that FYD could inhibit RCFibs proliferation. And FYD+Captopril could inhibit RCFibs proliferation more significantly than other groups (P<0.05or P<0.01).This inhibition effect shows in a dose dependent manner. Conclusion:FYD could inhibit the circular and local RAAS system, regress the proliferation of RCFibs, restrain early left ventricular expansion on the myocardial fibrosis, and improve early LVR after AMI. It might be the mechanism for FYD.
