ObjectiveFacing the current situation of the prevention and treatment of coronary heart disease, Controlling Chinese Medicine in preventing and treating Coronary Heart Disease, developing Chinese Medical advantages in treating potential disorders and Holism, basing on the recordation of thoracic obstruction Treatise on Febrile and Miscellaneous Diseases studying deeply the essence of the theory about the prevention and treatment of thoracic obstruction, we draw conclusions and put forward Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction. According to my supervisor's several years clinical experiences to control and prevention of cardiovascular diseases, directing by Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction, we made a clinical and experimental study of warming and obstruction-removing therapeutic methods by Chinese Herbs Tongyang Kuanxiong decoction. We explored Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction reflecting more really and actually the pathogenesis of thoracic obstruction. We explored that warming and obstruction-removing therapeutic methods directed by Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction was An Important treatment to thoracic obstruction MethodsThis study included three parts:the TCM literature studies, animal experiments and clinical trials.The objection of the TCM literature studies was in drawing conclusions and putting forward Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction. emphasizing Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction reflected more really and actually the pathogenesis of thoracic obstruction. It can reveal the development and progression and therapeutic mechanism of thoracic obstruction. We studied the TCM literatures of past dynasties. We analysis the Ancient Doctor's experience of treating thoracic obstruction, We concluded the theory of cause and pathogenesis in thoracic obstruction and explored of law of treatment based on syndrome differentiation, We think Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction is a system to direct the treatment of thoracic obstructionThe objection of animal experiments was proved that the efficacy of the warming and obstruction-removing therapeutic methods directed by Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction is superior to the method of promoting blood circulation by removing blood stasis. We made the modeling based on atherosclerosis'theory in Modern Medicine.40 rats were randomly divided into four groups:the group of blank model, the group of atherosclerosis model, experiment group and control group. The experiment group was gived Tongyang Kuanxiong Decoction. The control group was gave GuanxiⅡdecoction. Before and after treatment we observed and recorded the related physiochemical indexes. It was including of four items of blood lipid, plasma nitric oxide (NO) and endothelin (ET)The objection of clinical experiments was proved that the efficacy of the warming and obstruction-removing therapeutic methods directed by Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction is superior to the method of promoting blood circulation by removing blood stasis.79 patients with thoracic obstruction were divided into test group (n=40) and control group (n=39) The course of treatment in test group was the basic treatment of CHD and Tongyang Kuanxiong Decoction. The course of treatment in control group was the basic treatment of CHD and GuanxiⅡDecoction. Before and after treatment we observed and recorded patient's general data, clinical symptoms of patients improved after treatment, Traditional Chinese Medical syndrome score, plasma nitric oxide endothelin and thromboxane. At last we provides the Statistical analysis of the studiesResultsWe concluded Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction based on the TCM literature studies. It can be summarized as:The essentiality of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction revealed the pathogenesis of Coronary Heart Disease. through pulse condition. It means yang insufficiency in upper jiao and Yin excess in lower jiao. Insufficiency of yang means root deficiency. It is including of Qi deficiency and yang deficiency. It further displays the syndrome of deficiency of both qi and yin or the syndrome of deficiency of both qi and blood. Endogenous excess of yin means branch excess. It is including of Qi stagnation, cold coagulation, turbid phlegm and blood stasis. It further displays the syndrome of Qi stagnation and blood stasis, Qi stagnation due to congealing cold and turbid phlegm obstructing in midstThe result of animal experiment shows that blood liquid can be changed in experiment group and control group. There were no significant changes in experiment group. (P>0.01). It shows that NO and ET can be changed in experiment group and control group, but there were no significant changes compared with the control group in the change of NO (P>0.05). There were significant changes compared with the control group in the change of ET (P <0.05)The result of clinical experiment shows that clinical signs and electrocardiograph can be changed in test group and control group, but there were no significant changes in experiment group. (P>0.01). There were significant changes compared with the control group in the effect of heart function (P<0.05). Tongyang Kuanxiong Decoction is distinctly superior to GuanxiⅡDecoction in the effect of heart function. There were no significant changes compared with the control group in the effect of LDL-C, HDL-C, NO and TXB2 (P>0.01或P>0.05), but There were significant changes compared with the control group in the effect of TC, TG and ET (P<0.05) At last, we compared the Traditional Chinese Medical syndrome score and the gross curative effects. Tongyang Kuanxiong Decoction is distinctly superior to GuanxiⅡDecoction in the improvement of such symptoms as chest distress (P<0.01). At the same time, Tongyang Kuanxiong Decoction is more superior to GuanxiⅡDecoction in the gross curative effectsConclusions1.Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction revealed the pathogenesis of Coronary Heart DiseaseFirst, the clinical experiences in the TCM literature of different times support Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction. Second, our studies showed that warming and obstruction-removing therapeutic methods directed by Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction is more effect to thoracic obstruction than others therapeutic methods. It can Indirectly show that the theory is proved. All these can prove the validity of Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction.Based on three results of the TCM literature studies, animal experiments and clinical trials, Zhongjing-zhang's theory of insufficiency of yang and endogenous excess of yin pathogens on thoracic obstruction revealed the pathogenesis of Coronary Heart Disease in the field of Traditional Chinese Medicine and Integrated Traditional Chinese and Western Medicine.
