Study On The Inflammatory Cytokines In Deep Vein Thrombosis And The Interventional Mechanism Of Chinese Herbs

Objective: To investigate the pathogenesis of deep vein thrombosis(DVT) from the view of inflammatory cytokine and interventional mechanism of Chinese Herbs on DVT, in order to provide objective basis for clinical diagnosis and treatment.Methods: 60 patients of DVT were divided into groups according to the course of diseases and the syndrome differentiation type of traditional Chinese medicine(TCM) respectively. Based on the course of diseases, it was divided into acute phase, lag phase, chronic phase and recurrence phase. Based on the type of TCM, it was divided into the syndrome of pathogenic dampness-heat(A), the syndrome of blood stasis and dampness(B) and the syndrome of phlegm-stasis(C), 30 health people as control. All patients and healths were determined blood serum levels of CRP, TNF-α, IL-1β, IL-6, IL-8, IL-10 by immunoturbidimetry and radioimmunoassy. In the experiment, 150 Wistar rats were divided into seven groups which included Hua Zhi Tong Mai tablets significant dose group and small dose group, Chuan Wang Xiao Yan tablets group, Fu Fang Dan Shen tablets group, model group, sham group, blank group. Inferior vena cava ligation rat was applied as models. Then to observe the pathological character, the serum levels of TNF-α, IL-6, IL-8 and the expression of the inflammation cells, TNF-α, IL-10, E-selectin and NF-κBP65mRNA in the vein walls.Results:①T he serum levels of inflammatory cytokines of patients were higher than those of the control(P<0.05,P<0.01), the serum levels of CRP and TNF-αwere correlated to severity of thrombosis;②T he serum levels of IL-10 in lag phase were higher than other groups(P<0.05,P<0.01);③T he serum levels of CRP, TNF-α, IL-6 and IL-8 in recurrent phase were higher than other groups (P<0.05,P<0.01);④T he serum levels of CRP, TNF-αand IL-6 in group A were higher than group B and C(P<0.01), thus the levels of IL-10 in group B were higher than group A and C;⑤Experiment shows that pathological changes in models were more severe than sham, higher levels of TNF-α, IL-6, IL-8 and higher expressions of the inflammation cells, TNF-α, IL-10, E-selectin and NF-κBP65mRNA in the vein walls(P<0.05,P<0.01). The two Hua Zhi Tong Mai group can effectively improved the pathological changes, reduced the expressions of these inflammatory cytokines(P<0.05,P<0.01).Conclusions: The inflammatory cytokines may participate in all pathological courses of DVT which involved in genesis, development, organization and recanalization. It can be as one of the objective index in the syndrome type of TCM. The mechanism of the treatment of DVT by Chinese herbs may be the intervention and modulation of the inflammatory cytokines.
