Economic Analysis Of Chinese Health Care Reform

Health economics developed rapidly .today's research about health care system is devided into theoretical research and empiristic research.Health economitists abroad have done much in theoretical research and have brought it into mainstream economics framework, as well as they pay attentiont to empiristic research.This article take Chinese health care reform as a background, discuss its pricing system,payment system, production efficiency of hospital market, and the social efficiency of it.Health care system of a country contains three parts: Health care financing system(health insurance system);service delivery system; monitoring and supervising system.Even in those countries market economics dominant,their health care system differe lot.For exemple, there are National Health Service in England; commercial insurance in USA;the combination of individual account and social health insurance in Singapore.This paper constructed a model about the fixing of price level and the payment system for health services.In the regime before reform, government enact price at P=MC. In the regime after reform, government abandon the right of pricing and hospitals set price to maximize his utility.The welfare of former regime is higher. The way government subsidize consumers is also one of important factors that influence social welfare. Before reform the government carries on the subsidy to consumers' medical payment, they expense excessively in that they needn't share any cost. This payment system provide less financial incentives for consumers.By using data envelopment analysis,we measure hospital efficiency retrospectively during health care reform,shows that efficiency measures of hospital of different types descended during these years.Then we estimated pertinence relation between these efficiency value and the reduction of funds by government. Concluds that: among these hospitals, hospital efficiency value of county and higher-ranking hospitals has positive correlation with the reduction of government funds ,as well as those rural health centers. There are many factors that would influence the technical efficience of hospitals,economical feactures of health services determine that public sectors is efficient to provide health services.A suitable health care financing is progressive,that means a family with more income would pay more in health care financing. In contrast to this.Chinese health care financing in our empiristic research is regressive. After calculating Theil indexes of allocation of health sector,hospital beds and health technical personnel,we show that the indexes of allocation of health sector.hospital beds increase with years while that of health technical personnel decrease. This paper also analysis ineqity changes for utilization of health services with data of "An analysis report of National Health services Survey in 2003", then carried on the generalized linear regression analysis on the income and the time which shows that the effect become statistically significant in those teams.Many countries brought about health care reform in recent decades. The point of the health care reform in England is to introduce competition in NHS by establishing a "Internal market". Around 1983, Americal health care system which is market-orient,introduced managed care in order to control the rapid growth of cost.After that managed care adopted prospective payment system instead of "cost based reimbursement", which changed incentive to hospital.In Poland and Hungary,both of which are in transitive period, their reform of public hospital perform a series of internal governments reform such as financing and human affairs, and hold the extremely discrete manner to medical sector's privatization.The policies we conclude from this article are: Price should be controled by government ;While government pay for expenditure of health services,it will be better to increase level of consumers' out-of pocket expenditure;we should study American whose medical service organizational reform successfully carries restraint on the health service supplies side, and supplies side accept the medical service compensating level, not opposite; The government should increase the investment in hospital systemt, increases the investmen in health expenditure;We emphasize that "equity dominate efficience" in health care system;It will take a long time to cultivate a better health care market, The health system transition is a long-term process.its span could be very long.
