| Fresh-frozen cadaveric knees were tested to examine the effects of patellar resurfacing on patellofemoral joint biomechanics changes in total knee arthroplasty, and to determine the proper highness of joint line in TKA, then tibial plateau-fibular head distance was measured on anterioposterior view of knee roentgenograms. The results pointed out clearly that not resurfacing patella might be considered in selected patients with mild or no patellar arthritis, and particularly if a patella-friendly femoral component is used. And the variation of joint line should be controlled beyond 4mm in TKA. In the end, the distance between tibial plateau and fibular head should be routinely measured before TKA. If the distance is below eight millimeters, enough attention should be paid to protect the lateral collateral ligament when performing proximal tibial rescetion. |
