| [BACKGRUUND]Acute renal failure(ARF) is the frequent clinical emergency case. The most causes of the ARF are acute renal ischemia-reperfusion injury aroused by haemorrhage; sever injury; shock; kidney transplantation and so on. Although the application of the intensive care and blood purification have provided the advanced methods of treating ARF, which has made the patients' prognosis better, the mortality of AFR is still very high. So how to elevate prevention and cure level is still the investigative emphasis.The mechanism of the acute tubular necrosis (ATN) caused by acute renal ischemia-reperfusion injury is complicated and there are many correlation factors in it. The recent researches show that the oxygen free radical , some inflammatory medium such as tumor necrosis factora (TNF-a) and interleukin-1 (IL-1) , adhesion molecule 1 (ICAM-1) and vascular cell adhesion moleculel (VCAM-1), inflammatory cell infiltration are all related to the renal ischemia-reperfusion injury ,especially the effect of the oxygen free radical. During the ischemia-reperfusion injury ,the reperfused tissues generate a great deal of oxygen free radical through the paths of activating xanthine oxidase, NADPH oxidase and the other paths .The oxygen free radical not only injury the reperfused tissue directly, but also enlarge the injury effect of the oxygen free radical through peroxidation(oxidative stress).
