Object To investigate the expression and regulation of MHC class II molecule in MODS with treatment of HVHF. Methods Twenty-eight male pigs were randomly assigned into control group (group M, n=9), experimental group (group C, n=9) and therapeutic group (group HF, n=10) which were given operation, two-hit, and HVHF, respectively. We monitor the function of organs and the level of TNF-α and IL-10. After seven days the living animals were killed and spleen were used to perform RT-PCR of SLA-DQA mRNA and CIITA mRNA. CIITA protein was also detect by immunohistochemistry. Results The incidence rate and mortality of group M was significantly higher than which of group C and HF. The organs function was much better and the concentration of TNF-a and IL-10 presented decline after HVHF. The expressing quantity of SLA-DQA mRNA and CIITA mRNA of group M was significantly lower than that of group C (P<0.05). conclusions HVHF can efficiently decrease the level of pro-inflammatory and anti-inflammatory mediators. CIITA gene controlled transcription of MHC II gene, and the decrease of CIITA gene down-regulated expression of MHC class II gene. MHC II gene could be up-regulated by HVHF. There was relationship between MHC II molecule and TNF-a and IL-10. HVHF was an effective prophylactic treatment method in MODS by regulating immune balance.
