Objective To investigate the change and significance of vascular endothelial cell function in prophylactic treating multiple organ dysfunction with high volume hemofiltration(HVHF). Methods 18 pigs were randomized into two groups: group M as control group accepted "two hit" injury only, group HF as treatment group, proceeded with HVHF after "two hit" injury. The serum concentration of TXB2, 6-keto-PGF1a, sE-selectin and sICAM-1 were tested by ELISA. The normal porcine iliac artery endothelial cells (PIEC)were cultured in vitro, which had accepted the intervention of the serum collected from two groups porcine. The apoptosis state of PIEC was evaluated . Results The functions of major organ in group HF improved significantly. MODS morbidity and mortality of group HF were lower than that of group HF. (P<0.01) The concentration of TXB2 ,sE-selectin and sICAM-1 but 6-keto-PGFla were reduced by HVHF evidently (P < 0.05) .Having been compared with group M, the serum samples collected after HVHF intervention in group HF deduced lower apoptosis rate of PIEC calculated by flow cytometry , less mRNA expression of caspase-3 detected by RT-PCR and more protein expression of bcl-2 detected by western blot. (P<0.01) . Conclusion HVHF ,as an approach for preventing and treating MODS, could attenuated the peak concentration of pro-inflammatory and anti-inflammatory mediators and restore the immune-equilibrium. The function of vascular endothelial cells could be improved by HVHF effectively.
