The thickness of the nasal outer tissue is involved with the choice of implant graft in augmentation rhinoplasty. First choice to autogenous cartilage and bone as implant by western surgeons may be owing to ability of thin outer tissue envelop to take it .Jn order to verify the security and stability of the wildly used procedure with rigid silicone in augmentation rhinoplasty,it is necessaiy to understand the anatomic specifity of Chinese.The alar cartilage is an important component of low nose and mainly attribute to the shape of the nose tip.The detailed anatomy of the alar cartilage is investigated by foreign researcher ,but we don抰 find the same cartilage抯 frame in our practice,so comparative anatomic analysis is can-led out.The results show that the nose outer tissur have different thickness on axis line,the most thin points exist in keystone and colummela-lobularbreakpoint ,respectively 2.67 ? 0.87mm and 2.67 ?0.62mm,the most thick points are pronasale and supertip respectively 4.60 ?0.98mm and 4.03 ?l.56mm,the total soft tissue is more thick than that of western people.From our anatomic work ,the alar cartilage are conceived of as a medial crus and lateral crus joined at the dome,there are no apparant so-called middle cms, which is not a determint factor for the shape of nose tip.The alar cartilage is truely thin,about lnumThe above-mentioned results may used for guild to dissection and implant design in rhinolpasty, it also make us perceive that popular procedures of the nose tip may be not applicable to Chinese person. Subsequently, we propose a new combined implant which is composed of an ePTFE cap on angle site of L-shaped silicone prosthesis used in augmentation rhinoplasty, expecting to dramately augment nose tip and to establish a three- dimensional structure for getting full tip appearace in avoiding long-term complications such as implant visibility and exposure. We have inserted such implants in 23 patient noses and have achieved ideal aesthetic tip which is assured by follow-up.
