| Objective:To investigate the current situation of elderly stroke patient activation and explore the related factors affecting patient activation of elderly stroke patients, and provide reference for improving patient activation of elderly stroke patients. Methods:Used the convenient sampling method. From January 2022 to December 2022,237 elderly stroke patients who met the inclusion and exclusion criteria in the Department of Encephalopathy of a third class hospital in Shenyang were selected as the research objects.Using patient activation measure, together with Chronic Disease Self-efficacy Scale,Hospital Anxiety and Depression Scale,Connor-Davidson Resilience Scale and Social Support Rating Scale for questionaire survey. After data collection,SPSS25.0 was used for data entry and analysis. The general data, patient activation,self-efficacy,anxiety and depression, resilience and social support scores were analyzed by descriptive statistics.Measurement data are described by mean±standard deviation or median and quartile, and counting data are described by frequency and composition ratio.T-test and analysis of variance were used to compare the differences of patient activation scores of elderly stroke patients with different characteristics.Pearson and Spearman correlation analysis were used to analyze the correlation between patient activation and self-efficacy,anxiety and depression, resilience, social support.Multiple linear regression was used to analyze the influencing factors of elderly stroke patient activation, and the difference was statistically significant(P < 0.05).Results:1.The minimum patient activation score of elderly stroke patients was 38.46,and the maximum score was 86.54,and the overall mean score of patient activation was(62.24±9.14).2.Univariate analysis showed that the differences of gender, age, education level, place of residence, average monthly household income, any other chronic diseases,self-care ability of daily life, and type of normal physical activity were statistically significant(P < 0.05).3.The results of correlation analysis show that patient activationare positively correlated with self-efficacy(r=0.615,P<0.01);patient activation were negatively correlated with anxiety(r=-0.533,P<0.01);patient acitvation were negatively correlated with depression(r=-0.521,P<0.01);patient activation were positively correlated with resilience(r=0.594,P<0.01);patient activation were positively correlated with social support(r=0.416,P<0.01).4.The results of multiple linear regression analysis show that education level(β=0.139,P=0.005),place of residence(β=-0.106,P=0.032),other chronic diseases(β=0.142,P=0.002),self-efficacy(β=0.315,P<0.001),anxiety(β=-0.145,P=0.049),resilence(β=0.257,P<0.001)and social support(β=0.121,P=0.015)are the important influencing factors of patient activation. After adjustment,R2 = 0.562 in the regression model. These factors can explain 56.2% of the variance of patient activation.F=21.170,P< 0.001,and the fitted equation has statistical significance.Conclusion:1.Patient activation of elderly stroke patients is generally at a medium level, which needs further improvement.2.The influencing factors of elderly stroke patient activation include:education level, place of residence, average monthly household income, any other chronic diseases,self-efficacy,anxiety,psychological resilience and social support. It is suggested that medical staff can formulate corresponding intervention programs from the above factors to improve patient activation level of elderly stroke patients. |
