ObjectivesTo investigate the current situation of post stroke fatigue in elderly stroke patients and investigate the influencing factors.To provide clinical staff with evidence to develop intervention plans for patients with post stroke fatigue.MethodsThis study adopted a cross-sectional study design.According to the uniform inclusion and exclusion criteria,convenience sampling method was used to select elderly stroke patients who were hospitalized in the neurology department of 5 tertiary care general hospitals in Lanzhou,Gansu Province from June 2022 to February 2023 to participate in the study.The survey instruments were a self-designed general information questionnaire,the Chinese version of the Fatigue Severity Scale(FSS),Mini-Mental State Examination(MMSE),and 5-Item Geriatric Depression Scale(SDS).GDS-5)and Mini Nutritional Assessment-Short Form(MNA-SF).SPSS26.0 software was used for statistical analysis of the final collected data,categorical data were described by frequency and percentage;continuous data were described by mean ±standard deviation if they conformed to normal distribution,and median and interquartile spacing were used for data that did not conform to normal distribution;univariate analysis of post-stroke fatigue in study subjects with different demographic and disease characteristics was performed using chi-square test and nonparametric test(Mann-Whitney U test)were analyzed;multifactorial analysis was performed using binary logistic regression for variables that were statistically different in the singlefactor analysis.Results(1)A total of 618 participants were included in this study.The prevalence of post stroke fatigue was 31.4%,and the score of post stroke fatigue was(3.87±1.28).The prevalence of post stroke cognitive impairment and post stroke depression was 43.4%and 43.7%,respectively.(2)Univariate analysis showed that: The gender,age,BMI,education level,cerebrovascular history,coronary artery disease,leukoaraiosis,carotid atherosclerotic plaque,stroke frequency,number of lesions,NIHSS score,ADL score,uric acid level,C-reactive protein level,homocysteine level,cognitive impairment,depression and nutritional status were related to the occurrence of post stroke fatigue.The difference was statistically significant(P < 0.05).(3)Multifactorial analysis showed that gender,history of cerebrovascular disease,coronary heart disease,leukoaraiosis,ADL score,uric acid level,homocysteine level,depression and nutritional status were factors influencing the post-stroke fatigue of the study subjects.Among them,female(OR=2.462,95%CI: 1.608~3.771),cerebrovascular history(OR=2.035,95%CI: 1.328~3.119),coronary artery disease(OR=1.915,95%CI: 1.164~3.149),leukoaraiosis(OR=1.612,95%CI:1.401~1.935),high uric acid level(OR=1.012,95%CI: 1.009~1.015),high homocysteine level(OR=1.099,95%CI: 1.062~1.138),and depression(OR=2.334,95%CI: 1.537~3.545)were risk factors for post-stroke fatigue;high ADL score(OR=0.982,95%CI: 0.970~0.995)and normal nutritional status(OR=0.417,95%CI: 0.213~0.818)were protective factors for post-stroke fatigue.Conclusions(1)The high prevalence of post-stroke fatigue in elderly stroke patients should be of great concern to health care professionals.(2)Female,cerebrovascular history,coronary artery disease,leukoaraiosis,high uric acid level,high homocysteine level and depression are the risk factors of poststroke fatigue in elderly stroke patients,while high ADL score and normal nutritional status are protective factors of post-stroke fatigue in elderly stroke patients(3)It is recommended that screening for post-stroke fatigue be included in the routine screening program for hospitalized stroke patients to help clinical staff identify patients with post-stroke fatigue early and provide evidence to support the development of targeted treatment and care plans and interventions in the future.
