Objective:To study the clinical efficacy of Zhuyu Zhitong decoction combined with sacral canal injection in the treatment of lumbar disc herniation(qi stagnation and blood stasis syndrome).Methods:90 patients with lumbar disc herniation who meet the diagnostic criteria of traditional Chinese and western medicine and TCM syndrome types were selected and divided into treatment group(combined group),control group A(sacral canal injection group)and control group B(traditional Chinese medicine group)with 30 cases in each group.On the basis of basic treatment,the control group A was given sacral canal injection alone,while the control group B was given Zhuyu Zhitong decoction alone.The treatment group used the combination of the two therapies,with two weeks as a course of treatment,with a total of two courses.After each course of treatment,the JOA score,VAS score and TCM symptom score of the three groups before and after treatment and between the three groups were compared by statistical analysis,and the clinical efficacy and side effects of the three groups were objectively evaluated.Results:(1)Before treatment,there was no statistical difference in JOA,VAS score and TCM symptom score among the three groups(P > 0.05),but after one course of treatment and two courses of treatment,there were differences in JOA,VAS score and TCM symptom score between the three groups(P < 0.05);(2)After a course of treatment,the comparison among the three groups: the JOA and VAS scores of the combined group were better than those of the sacral canal injection group and the traditional Chinese medicine group(P < 0.05),and there was no statistical difference between the two groups in the control group(P > 0.05);In TCM symptom score,the combined group was better than sacral canal injection group and TCM group(P <0.05).Traditional Chinese medicine group was better than sacral canal injection group(P < 0.05).(3)After two courses of treatment,the symptoms of the three groups were further improved.In terms of JOA and VAS scores,the combined group was better than the sacral canal injection group and the Chinese medicine group(P < 0.05),and there was no statistical difference between the two groups in the control group(P > 0.05).In improving TCM symptom score,the combined group was superior to the sacral canal injection group and the Chinese medicine group(P < 0.05),and the Chinese medicine group was superior to the sacral canal injection group(P < 0.05).(4)Comparison of curative effect: the combined group was 96.55%,the sacral canal injection group was 82.76%,and the traditional Chinese medicine group was 86.67%.The combined group was higher than the control group(P < 0.05),and there was no statistical difference between the sacral canal injection group and the traditional Chinese medicine group(P > 0.05).(5)No serious adverse events occurred in the three groups.Conclusion:Combined group,sacral canal injection group and traditional Chinese medicine group can improve the symptoms of low back and leg pain,lumbar motion function and daily activities of patients with lumbar disc herniation(qi stagnation and blood stasis syndrome)to some extent;There was no significant difference in the overall curative effect between sacral canal injection group and traditional Chinese medicine group.Combined application of Zhuyu Zhitong decoction and sacral canal injection therapy in the treatment of LDH has the advantages of short onset time,definite curative effect,obvious advantages,safety and reliability,and is worthy of further research and application.
