| Background:In recent years,arthroscopy is becoming more and more mature,and its characteristics such as less trauma and remarkable effect make it more and more favored among doctors and patients in the surgical treatment of knee diseases.But postoperative knee swell and pain not only make patients miserable,but also affect patients’ postoperative recovery and functional exercise.The knee swelling and pain after the knee arthroscopy for meniscus,cruciate ligament injury and other diseases can be classified as "edema disease" and"tendon injury" in traditional Chinese medicine.Traditional Chinese medicine has a long time of accumulation and experience in the treatment of "edema disease" and "tendon injury".Numerous clinical trials and studies have proved that the method of Huoxue Lishui through traditional Chinese medicine can significantly relieve swelling and pain after arthroscopy of knee joint.The purpose of this study was to explore the therapeutic effect of Modified Danggui Shaoyao Powder,which also has the effect of Huoxue Lishui,on swelling and pain after arthroscopic knee surgery,in order to further expand the clinical application of Danggui Shaoyao powder and provide a new method for traditional Chinese medicine treatment of swelling and pain after arthroscopic knee surgery.Objective:To investigate the effect of Modified Danggui Shaoyao Powder on swelling and pain after arthroscopy of knee joint by clinical randomized trial.Methods:A total of 60 patients with knee swelling and pain after unilateral arthroscopic knee surgery due to meniscus or cruciate ligament injury in the orthopedic ward of Oriental Hospital of Beijing University of Chinese Medicine from March 2022 to March 2023 were collected and randomly divided into control group and treatment group,with 30 cases in each group.The control group received conventional symptomatic treatment on the day after surgery,such as lifting the affected limb,preventing infection,bandaging with elastic bandage,and indwelling drainage tube.On the day after surgery,the elastic bandage was removed,the drainage tube was removed,the wound dressing was changed routinely,and the lower limb functional exercise was guided after surgery.On the basic of the control group,the treatment group was combined with Modified Danggui Shaoyao powder granules,one dose a day,a total of 14 doses.Swelling values and VAS pain scores at the 3rd,7th and 14th day after surgery were analyzed.Results:Before treatment with Chinese medicine,there were no significant differences in swelling and VAS scores on day 1 after operation between the two groups(P>0.05).In terms of swelling value,the swelling value of the two groups on the 3rd day after surgery was significantly higher than that of the 1st day after surgery(P<0.05),but there was no significant difference between the treatment group and the control group on the 3rd day after surgery(P>0.05);In order to exclude the interference of drainage tube on day 1 after surgery,the swelling values in the group on day 7 and 14 after surgery were compared with those on day 1 and day 3,respectively.The swelling value of the control group on the 7th day after surgery was significantly increased compared with the 1st and 3rd day after surgery(P<0.05).The swelling value of the treatment group on day 7 after surgery had no significant difference compared with day 1 after surgery(P>0.05),but significantly decreased compared with day 3 after surgery(P<0.05).Compared with the two groups on the 7th day after surgery,the swelling value of the treatment group was significantly reduced than that of the control group,suggesting that the therapeutic effect of the treatment group was better than that of the control group on the 7th day after surgery.The swelling values of the control group on the 14th day after surgery were significantly higher than those on the 1st and 3rd day after surgery(P<0.05),indicating that the control group had no significant effect on the swelling on the 14th day after surgery.The swelling value of the treatment group on the 14th day after surgery was significantly decreased compared with the 1st and 3rd day after surgery,and showed a decreasing trend from the 3rd,7th,and 14th day after surgery.The swelling value of the treatment group on the 14th day after surgery was significantly decreased compared with the control group(P<0.05),indicating that the swelling of the treatment group on the 14th day after surgery was significantly better than the control group.In terms of VAS score,the VAS score on the 3rd,7th,and 14th day after surgery of the two groups was significantly lower than that on the 1st day after surgery(P<0.05),and showed a gradually decreasing trend,indicating that both groups could effectively relieve postoperative pain.Comparison showed no significant difference in VAS score on the 7th and 14th day after surgery between the treatment group and the control group(P>0.05),and the VAS score on the 3rd day after surgery was significantly lower than that of the control group,indicating that the therapeutic effect of the treatment group on the pain on the 3rd day after surgery was significantly better than that of the control group,while there was no significant difference on the 7th and 14th day after surgery.Conclusion:The addition of Modified Danggui Shaoyao Powder has a significant effect on early and middle stage swelling and early pain after arthroscopy of knee joint with meniscus and cruciate ligament injuries without obvious adverse reaction.The curative effect of the method of Huoxue Lishui in the early swelling and pain after arthroscopy of knee joint is worthy of affirmation. |
