| Research purpose:To evaluate the effect of abdominal acupuncture therapy on anxiety and depression in post percutaneous coronary intervention(PCI)patients with anxiety and depression,and to provide evidence for drug free therapy of traditional Chinese medicine for the treatment of this disease.Research Methods:1 A total of 60 patients conforming with the experimental standard randomly divided into intervention group and control group,with 30 patients in each group.2 On the basis of secondary prophylactic drugs for coronary heart disease(CHD),the intervention group received abdominal acupuncture treatment 3 times a week for 2 weeks,each time with infrared irradiation and needle retention for 30 minutes;The control group was treated with conventional secondary prophylactic drugs for CHD without other intervention and observed for 2 weeks.3 The following observation measures were recorded before and after the study period:General Anxiety Disorder(GAD-7)score,Patient Health Questionnaire(PHQ-9)score,TCM syndrome score,Pittsburgh Sleep Quality Index(PSQI)score,Seattle Angina Questionnaire(SAQ)score.4 The total effective rate was calculated by the reduction rate of GAD-7 and PHQ-9,and the curative effect was evaluated.5 Abnormal changes of safety indexes and adverse reactions were recorded during the study,and the safety of the intervention method was evaluated.Results:1 This study included a total of 63 patients with post-PCI anxiety and depression,who met the criteria and were admitted to the second ward,third ward and general outpatient department of Tongzhou Hospital,Dongzhimen Hospital,Beijing University of Chinese Medicine from October 2022 to May 2023.In the abdominal acupuncture group,32 patients were enrolled,2 patients were shed,and 30 cases were actually completed;The control group included 31 patients,with 1 case terminated,30 cases were completed.2 Compared with the control group,the intervention group had better effect on improving anxiety and depression.3 GAD-7:The overall response rate in the intervention group was 92%;The total effective rate of the control group was 37.5%;After treatment,there was a statistically significant difference between groups(P<0.001),and the reduction rate of self-rated anxiety score in the intervention group was better than that in the control group(P<0.001).PHQ-9:The total effective rate of the intervention group was 88.89%;53.85%in control group;After treatment,there was a statistically significant difference between groups(P<0.001),and the reduction rate of self-rated depression score in the intervention group was better than that in the control group(P<0.001).The results suggested that abdominal acupuncture could improve anxiety and depression in patients with coronary heart disease after PCI.4 TCM syndrome score:the total effective rate of intervention group was 80%;The control group was 13.3%,(P<0.001),the two groups have a significant difference in efficacy.PSQI:There was no significant difference before and after treatment in the control group(P>0.05),and there was a significant difference before and after treatment in the intervention group(P<0.05).The results indicated that after 2 weeks of treatment,abdominal acupuncture improved the pathological symptoms,sleeplessness and sleep quality better than the intervention group.5 Seattle Angina Questionnaire:There were no significant differences in the degree of physical limitation(PL)between the two groups before and after treatment(P>0.05).Angina stability(AS)and angina frequency(AF)in both groups were significantly different before and after treatment(P<0.05).On this basis,the intervention group showed significant differences in treatment satisfaction(TS)and disease perception(DS)before and after treatment,P<0.05.There was no significant difference between the intervention group and the control group in PL and AF before and after treatment(P>0.05);But before and after treatment on dry AS,TS and DS,the curative effect of intervention group was better than that of control group(P<0.05).6 Safety evaluation:Blood routine,stool routine,liver and kidney function,blood electrolyte test,electrocardiogram and so on before and after treatment were not significantly abnormal in two groups.One patient in the intervention group developed suspected alcohol-allergic rash after the first acupuncture without other allergic symptoms,the rash did not worsen after iodophor disinfection,and the rash subsided 2 days later,and the other patients did not have serious adverse reactions.Conclusion:Abdominal acupuncture therapy can effectively improve the anxiety and depression of post-PCI patients with anxiety and depression,and effectively improve the accompanying pathological symptoms such as angina pectoris,physiological discomfort and other TCM syndrome and sleep quality.Besides,it is painless,safe,convenient and has no major adverse events,so it can be further applied in this patient group. |
