| Objective:To observe the clinical efficacy of the Adipose-derived stem cells group and the combination of Zhitong Jiangu Decoction with Adipose-derived stem cells in the treatment of osteoarthritis of the knee(phlegm-stasis blocking the ligament and liver-kidneydeficiency type).Methods:Fifty-nine patients who met the diagnostic criteria of Knee Osteoarthritis(KOA)in the 2018 osteoarthritis treatment guideline and the type of knee paralysis with phlegm and blood stasis blockage and liver and kidney deficiency were randomly divided into three groups: the basic treatment group,the adipose stem cells group,and the combination treatment group,of which 20 cases were in the basic treatment group,20 cases in the Adipose stem cell group,and 19 cases in the combination treatment group.The basic treatment group was treated with oral glucosamine sulfate capsules and sodium glacial injection in the knee joint;the adipose stem cell group was treated with autologous abdominal adipose tissue,which was isolated and cultured to produce adipose stem cells and then injected into the knee joint;the combined treatment group was treated with Zhitong Jiangu Decoction granules orally on top of the adipose stem cell group.The basic data of the patients were recorded,and the improvement of the visual analogue scale of pain(VAS score),WOMAC score of osteoarthritis index scale and TCM symptom score were observed in the three groups before treatment,1month after treatment,2 months after treatment,3 months after treatment and 6months after treatment.All observed data were statistically analyzed using the statistical software SPAA26.0.Results:Within-group efficacy analysis: the three groups showed significant differences in VAS score,WOMAC score,quantitative TCM symptom score,and TCM effective rate at 1,2,3,and 6 months after treatment compared with those before treatment(P<0.05).Between-group efficacy analysis:(1)VAS scores: There were differences in the scoring trends among the three groups.2,3 and 6 months after treatment,the adipose stem cells group and the combination treatment group had significantly lower scores than the basic medication group(P<0.05);3 and 6 months after treatment,the combination treatment group had significantly lower scores than the adipose stem cell group(P<0.05).(2)WOMAC score: there was a difference in the trend of the scores of the three groups,and the scores of the adipose stem cells group and the combined treatment group were significantly lower than those of the basic medication group at 3and 6 months after treatment(P<0.05).(3)Quantitative TCM syndrome score: there was a difference in the scoring trend among the three groups,2 months after treatment,the score was significantly lower in the combined treatment group compared with the basic treatment group(P<0.05);after treatment,3 and 6 months,the score was significantly lower in the adipose stem cells group and the combined treatment group compared with the basic medication group(P<0.05);3 and 6 months after treatment,the score was significantly lower in the combined treatment group compared with the adipose stem cells group(P<0.05).(4)Effective rate: 3 months after treatment,there was no significant difference in the effective rate of the three groups(P>0.05);6months after treatment,the combined drug group was better than the adipose stem cells group and the basic treatment group,and the adipose stem cells group was better than the basic treatment group(P<0.05).Safety evaluation: during the treatment period,no reaction was observed in the basic treatment group;local pain was observed in a total of 7 cases in the adipose stem cell group and 4 cases in the combined drug group,which subsided on their own after 3-5 days;no cases of fever,infection,immune exclusion reaction,cancer,or death were observed during the follow-up period.Conclusion:Intra-articular injection of autologous adipose stem cells and Zhitong Jiangu Decoction combined with adipose stem cells can effectively reduce patients’ pain and improve patients’ mobility and TCM-related symptoms,and the effect of Zhitong Jiangu Decoction combined with the adipose stem cells was more obvious than that of adipose stem cells alone in VAS and TCM symptom scores at 6 months after treatment. |
