| Objective:To investigate the effects of type 2 diabetes mellitus(T2DM)on sperm acrosome reaction(AR)rate and sperm mitochondrial membrane potential(MMP)in patients.Methods:A total of 51 patients with T2DM who visited Zhejiang Mater Child and Reproductive Health Center from September 2018 to February 2022 were screened,and 33 men of childbearing age who came to the center for health examination during the same period were selected as the control group.Collect the patient’s blood and semen,detect fasting blood glucose,glycated hemoglobin(Hb A1c),reproductive hormones(LH,FSH,PRL,T,E),and semen analysis,sperm acrosome reaction rate(AR)and sperm mitochondrial membrane potential(MMP).Compute sperm deformity index(SDI)and teratozoospermia index(TZI).The Pearson or Spearman method was used to analyze the relationship between sperm AR,MMP and semen parameters in the T2DM group.By comparing the differences of the above indicators between the two groups,the effect of T2DM on male fertility would be investigated.Results:(1)Comparison of general information,fasting blood glucose,Hb A1c and reproductive hormones between T2DM group and control group:fasting blood glucose and Hb A1c in T2DM group were significantly higher than those in control group(P<0.001);The level of E2 was significantly higher than that of the control group(0.001<P<0.05).The level of T was significantly lower than that of the control group(0.001<P<0.05),and there were no significant differences in FSH,LH and PRL values between the T2DM group and the control group(P>0.05).(2)Comparison of routine parameters and morphology of semen between T2DM group and control group:The semen volume,p H,sperm PR,sperm PR,sperm PR+NP,and normal morphological percentage of sperm in the T2DM group were significantly lower than those in the control group(P<0.001),the rate of sperm head defect,sperm neck or middle defect was significantly higher than that of the control group(P<0.001),TZI and SDI were also significantly higher than that of the control group(P<0.001).There was no significant difference in sperm concentration and sperm tail defect rate between the two groups(P>0.05).(3)Comparison of sperm AR and MMP in T2DM group and control group:Sperm AR and MMP in T2DM group were lower than those in control group,and sperm AR was statistically significantly different(P<0.001),while sperm MMP in the two groups did not differ at the significance level of 0.05.(4)Sperm AR in T2DM group was positively correlated with the percentage of forward motility sperm and sperm motility(P<0.001);Sperm MMP was positively correlated with the percentage of normal morphology of sperm(P<0.05).Conclusions:(1)T2DM can reduce male T level and increase E2 level,which may affect sperm production.(2)T2DM will reduce male semen volume,p H,sperm PR,sperm PR+NP and the percentage of normal morphology of sperm,which may affect male fertility.(3)The AR rate of sperm in T2DM patients was significantly reduced,and the decrease in AR rate is related to the decline of progressive motility sperm and sperm motility,which may be one of the causes of infertility.There is no significant change in sperm MMP in patients with T2DM,which may be related to the course of DM,and the relationship between the two needs be investigated in future. |
