Objective: To study and compare the effects of levetiracetam(LEV),lamotrigine(LTG),topiramate(TPM),a new type commonly used anti-seizure medications(ASMs)and traditional drugs magnesium valproate(VPM)and carbamazepine(CBZ)on trace elements in whole blood of patients with epilepsy.The purpose is to provide guidance for the understanding of the etiology of epilepsy,the difficulty of treatment and prevention decision-making.Methods: Collect and analyze epilepsy patients who were treated in the neurology outpatient department of the Affiliated Hospital of Guilin Medical College from March 2017 to October 2021.A total of 113 cases aged 18-80 who met the diagnostic criteria of the International League Against Epilepsy in 2017 were screened.In addition,20 healthy subjects or volunteers in our hospital were selected as the control group.The enrolled patients were given corresponding drug treatment according to their seizure type and related examination results.A total of 5 groups of single ASMs treatment group and1 group of combination treatment.Each group of single-agent LEV,LTG,TPM,VPM,CBZ were treated for at least 6 months.The whole blood level of trace elements Iron,Zinc,Copper,Calcium,Magnesium and Lead were detected by automatic trace element analyzer(Shandong Qili QL8000C).Results:1.Comparison of whole blood trace elements detection results of the epilepsy group and the healthy control group before treatment: the whole blood Calcium ion of the epilepsy group was significantly lower than that of the healthy control group(P<0.05).There was no significant difference in the whole blood Copper,Zinc,Magnesium,Iron and Lead ions between the epilepsy group and the healthy control group.2.Comparison of whole blood trace elements detection results of epilepsy patients before and after single medication treatment: the Lead content of the VPM group after medication was significantly higher than that before medication,while the Copper ion content after medication was significantly lower than that after treatment(P<0.05);the Zinc content of LEV group after treatment was significantly lower than that before treatment(P<0.05).There was no significant difference in the content of whole blood Copper,Zinc,Calcium,Magnesium,Iron and Lead in of CBZ group,LTG group and TPM group before and after treatment.3.Comparison of trace element detection results in ASMs monotherapy group: the Calcium content of all monotherapy groups was significantly lower than that of the control group(P<0.05);The Copper ion content of the VPM group was significantly lower than that of the control group,LTG group and LEV group(P<0.05);The Zinc ion content of the VPM group was significantly lower than that of the control group(P<0.05);The Magnesium ion content of the CBZ group was significantly lower than that of the control group(P<0.05).The Iron ion concentration in the CBZ group was significantly higher than that in the LTG group and the TPM group(P<0.05),and the Lead content in the TPM group was significantly higher than that in the control group(P<0.05).4.The Magnesium ion content of patients using traditional ASMs and new ASMs was significantly higher than that of traditional combined new type ASMs users(P<0.05).There was no significant difference in the contents of Copper,Zinc,Calcium,Iron and Lead in whole blood between traditional ASMs,new type ASMs and combination groups.Conclusions: 1.Whole blood Calcium ion level is abnormal in epilepsy patients.Combined with the previous research results,it is speculated that the decrease of whole blood calcium ion level can promote the occurrence of epileptic seizures or the reduction of calcium ion level is the result of repeated seizures.2.Both the new type ASMs and traditional ASMs have certain effects on whole blood trace elements,and the three new type ASMs(LEV,LTG,and TPM)have less effect on trace elements than traditional ASMs VPM.It is recommended that clinicians should pay attention to the changes of the whole blood trace elements in epilepsy patients,and supplement and correction should be done when abnormality is found.
