Objective: To evaluate the clinical efficacy of spinal balance manipulation combined with cervical and lumbar pain tincture application in the treatment of lumbar disc herniation based on the theory of " musculoskeletal balance ".Methods: Using SPSS 25.0 random number method,62 LDH patients who were admitted to the outpatient and inpatient departments of the Second Department of Spine and Orthopedics of Gansu Provincial Hospital of Traditional Chinese Medicine from January 2022 to December2022 were randomly divided into an experimental group and a control group.Excluding the dropout cases in the experiment,30 patients were included in the experimental group,including15 males and 15 females,aged 18-65(47.23 ± 11.50)years old,with a course of disease of 8-22(14.27 ± 3.67)months;The control group included 30 patients,16 males and 14 females,aged 21-64(46.37 ± 13.20)years old,with a course of disease of 8-24(14.90 ± 3.82)months.The experimental group was treated with spinal adjustment technique combined with neck and low back pain tincture,while the control group was only treated with spinal adjustment technique.The spinal adjustment technique was performed once a week,and the neck and low back pain tincture was applied once a day,for a total of 3 courses of treatment.The VAS score,ODI score,and MJOA score of the two groups of patients were recorded before treatment,3weeks,1 month,and 3 months after treatment After 3 weeks of treatment,the inflammatory index IL-6 and pain substance SP in the patient’s serum were detected.At 3 weeks of treatment,the overall efficacy of the two groups was determined,and safety evaluation was conducted.After statistical analysis,the clinical efficacy of spinal balance adjustment technique combined with cervical and lumbar pain tincture application in the treatment of lumbar disc herniation and its impact on IL-6 and SP in the patient’s serum were analyzed.Results:(1)There was no statistically significant difference(P>0.05)between the two groups in terms of gender,age,course of disease,pre treatment VAS score,ODI score,MJOA score,IL-6,SP comparison,and baseline data was comparable.(2)The VAS score showed that there was statistical significance(P<0.05)in the comparison between the two groups before and after treatment for 3 weeks,1 month,and 3months(P<0.001).The comparison between the two groups was significant(P<0.001),indicating that both methods can alleviate pain symptoms in LDH patients.However,the combination of spinal balance manipulation and application of neck and back pain tincture has a better effect.(3)The ODI score showed that there was statistical significance(P<0.05)in the comparison between the two groups before and after treatment for 3 weeks,1 month,and 3months(P<0.001).The comparison between the groups was also significant(P<0.001),indicating that both methods can improve lumbar function and quality of life,and the improvement degree of spinal balance hand method combined with cervical and lumbar pain tincture application is greater than that of spinal balance technique.(4)The MJOA score showed significant differences between the two groups at 3 weeks of treatment,1 month after treatment,and 3 months after treatment(P<0.001).Intragroup comparison showed that there were significant differences(P<0.05)between the experimental group before treatment and 3 weeks,1 month,and 3 months after treatment.The control group showed significant differences before treatment,3 weeks,and 1 month after treatment,while there was no significant difference(P>0.05)compared to 3 months after treatment.This indicates that both methods can reduce the degree of low back pain and lower limb pain in patients,alleviate clinical signs,and improve daily behavioral activities,However,the combination of spinal adjustment technique and neck and back pain tincture was more effective than spinal adjustment technique at 3 month.(5)After 3 weeks of treatment,there were 24 cases of significant effect,4 cases of effective effect,and 2 cases of ineffective effect in the experimental group,with a total effective rate of93.3% and a recovery rate of 80%;The control group had 7 cases of significant effect,15 cases of effective effect,and 8 cases of ineffective effect.The total effective rate was 73.3%,and the recovery rate was 23.3%.There was no statistically significant difference in the total effective rate and recovery rate between the two groups(P<0.05),indicating that the overall efficacy of the experimental group was better than that of the control group.(6)The inflammatory indicators IL-6 and pain substance SP showed significant differences(P<0.05)within and between groups before and after 3 weeks of treatment,indicating that both methods can reduce the levels of IL-6 and SP.However,the reduction degree of spinal balance manipulation combined with cervical and lumbar pain tincture was higher than that of spinal balance manipulation.(7)The safety evaluation showed that there were no adverse events during the experiment,and there were no significant abnormal changes in the vital signs,electrocardiogram,and laboratory examinations of all subjects before and after the experiment.Conclusions:(1)The combination of spinal adjustment technique with neck and low back pain tincture or the application of spinal adjustment technique alone can improve the pain symptoms,clinical signs,lumbar activity function,daily behavior,and quality of life of patients with lumbar disc herniation.Both methods are safe and reliable,and have the prospect of clinical promotion and application.However,the combination of spinal adjustment technique with neck and low back pain tincture has better long-term efficacy and overall efficacy.(2)The combination of spinal balance manipulation with neck and back pain tincture or the application of spinal balance manipulation alone can reduce the levels of inflammatory factor IL-6 and pain substance SP in LDH patients,suggesting that the mechanisms of the two methods for treating LDH may be related to the downregulation of IL-6 and SP levels.
