Objective:In this study,a cross-sectional survey was conducted to understand the current situation of the quality of life of patients recovering from stroke and to analyze the relevant factors affecting the quality of life of patients;a nursing intervention program based on IKAP theory model of health education combined with Chinese medicine five-tone therapy was developed to evaluate the intervention effect in order to improve the negative emotions of patients recovering from stroke,reduce the sense of stigma,improve the quality of life of patients and promote their recovery.Methods:Part Ⅰ Cross-sectional survey: From February 2022 to June 2022,270 patients who met the inclusion criteria in the rehabilitation medicine department of a tertiary hospital in Lanzhou City were selected as the study population using a convenience sampling method.A self-administered general information questionnaire and a stroke-specific quality-of-life scale were used to conduct the questionnaire survey.SPSS 26.0 software was applied for statistical analysis.Statistical methods included: descriptive analysis,independent sample ttest,one-way ANOVA and multiple linear stratified regression analysis.Part Ⅱ Intervention study: From July 2022 to February 2023,70 patients who were hospitalized in the rehabilitation medicine department of a tertiary hospital in Lanzhou City during the stroke rehabilitation period were selected and divided into 35 cases each in the control group and the experimental group using randomized grouping,with the control group using conventional health education combined with TCM five-tone therapy and the experimental group using health education combined with TCM five-tone therapy based on the IKAP theory model at The Positive Negative Affect Scale(PANAS)scores,Stroke Stigma Scale(SSS)scores and Stroke-Specific Quality of Life Scale(SS-QOL)scores were compared between the two groups before,1 month and 3 months after the intervention.The data were statistically processed and analyzed using SPSS 26.0 statistical software.Statistical methods included: t-test,2-test,and repeated measures ANOVA,with P < 0.05 indicating statistically significant differences.Results:1.Cross-sectional findings:A total of 270 patients were included in this study,of which the lowest score for quality of life was 109 and the highest score was 184,with a total score of(158.91±12.46).The highest scoring dimension among the 12 dimensions included in the scale was language(19.88±4.17)and the lowest scoring dimension was work/labor(8.16±2.86).The results of univariate analysis showed that gender,age,education level,residence status,per capita monthly household income,number of comorbid chronic diseases,self-care status,and type of stroke onset were significant factors affecting the quality of life of patients during stroke rehabilitation(P < 0.05).Multiple linear regression analysis showed that a total of five variables,namely,education level,monthly per capita household income status,self-care status,and number of comorbid chronic diseases,entered the regression equation and together explained 56.1% of the total variance in quality of life in patients recovering from stroke(F=43.894,P<0.001)(1)Before the intervention,the differences in general information,stigma,positive and negative emotions,and quality of life scores between the two groups were not statistically significant(P>0.05)and were comparable.(2)After 3 months of intervention,the positive emotion and negative emotion scores of the subjects in the experimental group were significantly higher and lower than those in the control group,and the difference was statistically significant(P < 0.05);the stigma scores of the subjects in the experimental group were significantly lower than those in the control group,and the difference was statistically significant(P < 0.05);the quality of life scores of the patients in the experimental group were significantly higher than those in the control group,and the difference was statistically significant(P < 0.05).The difference was statistically significant(P < 0.05).(3)Repeated-measures ANOVA results showed that the differences in positive and negative emotions,stigma,and quality of life scores between the two groups were statistically significant(P < 0.05);there was an interaction between time and group factors for positive and negative emotions,stigma,and quality of life scores of patients(P < 0.05)Conclusion:1.The overall quality of life scores of patients in the stroke rehabilitation period in this study were at a moderate level and need to be further improved.2.Health education based on the IKAP theoretical model combined with Chinese medicine five-tone therapy can improve the negative emotions of patients in the stroke recovery period,reduce the sense of stigma and improve the quality of life level of patients,which provides a reference basis for promoting stroke recovery and clinical development of interventions for stroke patients’ quality of life.
