| Objective To analyze the incidence,mortality and disease burden of major malignant tumors in Baiyin City from 2017 to 2019 and the tendency of their changes.And to provide appropriate reference basis for the formulation of tumor prevention and control strategies.Methods This study collected all cases of malignant tumor incidence and death from2017 to 2019 in the "China Cancer Registration Platform" with the registered residence address of Baiyin City,and supplemented the accuracy and authenticity of the data through missing reports.Descriptive analysis of the incidence and mortality of malignant tumors by calculating indicators such as gender,urban and rural incidence/mortality.Calculated the disability-adjusted life years(DALY)and the DALY rate to estimate the disease burden of malignant tumors.Meanwhile,calculate the annual percentage change(APC)and its 95%confidence interval(CI)to analyze the trend of the incidence,mortality and disease burden of cancer.Results(1)The total population covered by the cancer registries in Baiyin City from 2017-2019 was 5,325,325 of which 2,740,515 were males and 2,584,810 were females.The urban population was 1,579,432 of which 819,702 were males and 759,730 were females,while the rural population was 3,745,893 of which 1,920,813 were males and 1,825,080 were females.(2)From 2017 to 2019,there were a total of 12,865 new cases of cancer in the cancer registration areas of Baiyin City(6,817 males and 6,048 females).The crude incidence rate was 241.58/100,000(248.75/100,000 for males and 233.98/100,000 for females).The age-standardized incidence rate by 2010’s Chinese standard population(ASIRC)was220.61/100,000.The age-standardized incidence rate by World Segi’s population(ASIRW)was 172.51/100,000.The cumulative rate of 0-74 years old were 19.79%.In general,the crude incidence,ASIRC and ASIRW for males were higher than those for females,In rural areas,the male ASIRC and ASIRW are higher than those of women,while in urban areas,the female ASIRC and ASIRW are higher than those of men.And the crude incidence,ASIRC and ASIRW in rural areas were higher than those in urban areas.The age-specific incidence of cancer was at a low level before the age of 40 years old,and rose rapidly after that,reaching a peak in the age group,from 80 to 84 years old.The top 5 of most common cancers were stomach cancer,lung cancer,liver cancer,colorectal cancer and female breast cancer in order.In total,breast cancer ranked first in women,lung cancer ranked first in urban areas,and stomach cancer ranked first in rural areas.Among the common cancers,there were huge disparities in the incidence between urban and rural areas.From 2017 to 2019,the overall incidence of cancer kept stable,incidence in urban males showed an upward trend with the APC of 13%(P=0.02).The incidence of liver cancer for rural males showed downward trends,with the APC of 9.6%(P=0.01),respectively.The incidence of lung cancer and esophageal cancer for urban males and cervical cancer for rural females showed upward trends,with the APC of 12.7%(P=0.04),19.5%(P=0.01),and 28.1%(P=0.04),respectively.(3)From 2017 to 2019,there were a total of 6,467 deaths of cancer in the cancer registration areas of Baiyin City(3,953 males and 2,514 females).The crude mortality rate was 121.44/100,000(144.24/100,000 for males and 97.26/100,000 for females).The age-standardized mortality rate by 2010’s Chinese standard population(ASIRC)was109.48/100,000.The age-standardized mortality rate by World Segi’s population(ASIRW)was 82.42/100,000.The cumulative rate of 0-74 years old were 9.17%.In general,the crude mortality,ASIRC and ASIRW for males were higher than those for females,the crude incidence,ASIRC and ASIRW in rural areas were higher than those in urban areas.The crude mortality rate for female in rural areas is lower than that in urban areas.The age-specific mortality of cancer was at a low level before the age of 45,and rose rapidly after that,reaching a peak in the age group of 80-84 years old.The top 5 of most common cancers were stomach cancer,lung cancer,liver cancer,colorectal cancer and pancreatic cancer in order.In total,lung cancer ranked first in women,lung cancer ranked first in urban areas,and stomach cancer ranked first in rural areas.Among the common cancers,there were huge disparities in the mortality between urban and rural areas.From 2017 to 2019,the overall mortality of cancer kept stable.The mortality of colorectal cancer for urban males showed downward trends,with the APC of 14.4%(P=0.04),respectively.(4)From 2017 to 2019,the overall DALY of cancer was 86,465 person-years(52,657person-years males and 33,852 person-years females),and the rate of DALY was 16.24/1,000(19.21/1,000 males and 13.10/1,000 females).The disease burden is mainly caused by early death,accounting for 98.58%.Males are 1.48 times as many as females.In general,the rate of DALY for males were higher than those for females,the rate of DALY in rural areas were higher than those in urban areas.The crude mortality rate for female in rural areas is lower than that in urban areas.The rate of DALY was at a low level before the age of 30 years old,and rose rapidly after that,reaching a peak over 70 years old group.The top 5 of most common cancers were stomach cancer,lung cancer,liver cancer,colorectal cancer and pancreatic cancer in order.In total,lung cancer ranked first in women,lung cancer ranked first in urban areas,and stomach cancer ranked first in rural areas.Among the common cancers,there were huge disparities in the rate of DALY between urban and rural areas.Conclusions From 2017 to 2019,the incidence rate,the mortality rate and the DALY rate of malignant tumors in Baiyin City was lower than the national average,and the trend of change in three years was basically stable.Gastrointestinal cancer,lung cancer and female breast cancer are the main malignant tumors that cause morbidity and mortality of residents.The medical expenditure should be increased,and the environment should be renovated.The health examination and screening in the male over 40 years old in rural area,early diagnosis and treatment for "two cancers" in the females aged from 40-65 years,the hepatitis B vaccination in whole population,and HPV vaccination in the females in rural area should be strengthened.The health education and promotion activities should be actively carried out. |
