Objective:In this paper,we will investigate the correlation between peripheral blood eosinophil-derived toxin(EDN),Ezrin and severity of disease in patients with acute exacerbation of bronchial asthma,and to provide serological markers for the diagnosis of acute exacerbation of bronchial asthma by single and combined testing of these indicators.Methods:A total of 60 patients(29 males and 31 females)who met the inclusion criteria of this study were selected as the observation group and 20 patients(10 males and 10females)who were admitted to the Department of Respiratory and Critical Care Medicine of the Second Clinical Medical College of Shanxi Medical University from February 2022 to February 2023 for acute asthma attacks were selected as the control group.The clinical data of the patients in the observation group were collected,including age,sex,height,weight,related symptoms,signs,arterial blood p H,arterial partial pressure of oxygen(Pa O2),arterial partial pressure of carbon dioxide(Pa CO2),arterial partial pressure of oxygen(SaO2),and peak expiratory flow rate(PEF)in the pulmonary function test.Based on the above indices,patients were divided into mild,moderate and severe groups(20 cases each)according to the severity of the disease.All subjects were tested for EDN,Ezrin,absolute peripheral blood eosinophil(EOS),total serum immunoglobulin E(Ig E),interleukin-13(IL-13)levels,pulmonary function indices(1st second expiratory volume on force(FEV1),force spirometry(FVC),FEV1/FVC),and exhaled nitric oxide(Fe NO).Spearman correlation was used to analyze the correlation between serum EDN and Ezrin levels and the severity of the disease in patients with acute exacerbation of asthma and to assess the diagnostic value of these two indexes for acute exacerbation of bronchial asthma by plotting ROC curves.The data were collated and analyzed using SPSS27.0 statistical software.Results:1.Age,sex,weight,height,and body mass index(BMI)between groups,P > 0.05,no statistically significant differences.2.Compared with the control group,the serum EDN level in the observation group was significantly higher than that in the control group(P < 0.05)and showed an increasing effect with the severity of the disease;while the serum Ezrin level was significantly lower than that in the control group(P < 0.05)and showed a decreasing effect with the severity of the disease,and the differences were statistically significant(P< 0.05).3.The peripheral blood EOS count,serum total Ig E level,IL-13,and Fe NO levels were significantly higher in the severe group than in the mild and moderate groups(P <0.05);the peripheral blood EOS count,serum total Ig E level,IL-13,and Fe NO levels were higher in the moderate group than in the mild group(P < 0.05);the FEV1,FVC,and FEV1/FVC were significantly lower in the severe group than in the The FEV1,FVC and FEV1/FVC of patients in the severe group were significantly lower than those in the mild and moderate groups(P < 0.05);each index of lung function decreased as the degree of acute asthma exacerbation increased.4.Spearman’s correlation analysis showed that serum EDN levels were positively correlated with the severity of bronchial asthma((rs value of 0.805,P < 0.05);serum Ezrin levels were negatively correlated with the severity of bronchial asthma(rs value of-0.836,P < 0.05).5.Pearson correlation analysis showed that serum EDN levels were positively correlated with peripheral blood EOS count,serum total Ig E,IL-13,and Fe NO(r values were 0.617,0.698,0.66,and 0.627,respectively,P < 0.05)and negatively correlated with FEV1,FVC,and FEV1/FVC(r values were-0.506,-0.463,-0.429,P < 0.05);serum Ezrin levels were negatively correlated with peripheral blood EOS count,serum total Ig E,IL-13,and Fe NO(r values of-0.682,-0.757,-0.717,-0.682,P < 0.05),and positively correlated with FEV1,FVC,and FEV1/FVC(r values of(0.536,0.494,0.447,P < 0.05).6.The AUC value of EDN was 0.780,and the sensitivity and specificity were 88.3%and 66.7%,respectively;the AUC value of Ezrin was 0.743,and the sensitivity and specificity were 91.7% and 55%,respectively;the AUC value of EDN-Ezrin was 0.845,and the sensitivity and specificity were 88.3% and 90%,respectively.Conclusion:1.The peripheral blood EDN level of patients with acute bronchial asthma exacerbation increases with the severity of the disease and the Ezrin level decreases with the severity of the disease,both of which can be used as clinical predictors of the degree of asthma disease.2.The combined detection of serum EDN and Ezrin levels has better diagnostic value for acute attacks of bronchial asthma,and can be used as an important marker for clinical auxiliary diagnosis of acute asthma attacks. |
