Objective:To investigate the fear of childbirth of single primiparas in the Department of Obstetrics,the First Hospital of Shanxi Medical University and the effects of fear of childbirth on pregnancy outcomes.The Internet group health management method was selected for health education of single primiparas,and the effects of this method were evaluated.Methods:1.From October 1,2021 to October 31,2022,617 single primiparas,who received regular birth check-up and gave birth in the Department of Obstetrics,the First Hospital of Shanxi Medical University were included as subjects.After receiving their informed consent,a questionnaire survey was conducted on pregnant women when they came to the hospital for obstetrical examination,and information about childbirth was obtained according to relevant information,including gestational week,delivery mode,postpartum blood loss,labor process,length of stay,etc.,to analyze the influence of different degrees of fear of childbirth on pregnancy outcome.2.Based on the review of literature,books,guidelines and cross-sectional survey results,based on the current social situation,combined with expert opinions and the physiological and psychological characteristics of pregnant women,an Internet-based health management intervention method was developed for clinical application.In the first part,200 pregnant women who received regular obstetrical check-up and delivery in the First Hospital of Shanxi Medical University were randomly selected as research objects.According to the principle of randomized control,they were divided into intervention group and control group.The control group received routine outpatient health education during childbirth examination,and the intervention group was given 5times of health education(once every 2 weeks)on the basis of this.The patients’ emotions and related pregnancy outcome evaluation indexes were compared and analyzed before and after intervention.Results:1.The effects of fear of childbirth on pregnancy outcome of single primiparas(1)In this study,a total of 617 single primiparas were investigated,and the incidence of fear of childbirth was 74.7%.With the deepening of fear of childbirth,the rate of natural childbirth gradually decreased,while the incidence of non-medical indications of caesarean section gradually increased,with statistical significance(P<0.05).(2)There were statistically significant differences among all groups in total gestation days,incidence of premature infants,2 hours postpartum bleeding,incidence of postpartum bleeding,Apgar score,neonatal weight,low birth weight infants,postpartum hospitalization days,and incidence of non-medical indications of cesarean section(P<0.05),which mainly affected the moderate and severe fear of childbirth group.There were no significant differences in the total labor course,the incidence of macrosomia,the rate of lateral perineal resection and the incidence of cesarean section transfer(P>0.05).2.The influence of Internet-based group health management on emotions and pregnancy outcomes of single primipara(1)Before intervention,there were no significant differences in general information,fear of childbirth,anxiety and depression between the two groups(P>0.05).(2)After intervention,the scores of fear of childbirth,anxiety and depression in the two groups;There were statistically significant differences in anxiety and depression at 6weeks postpartum(P<0.05).(3)After intervention,there were statistically significant differences between the two groups in gestation days,natural delivery rate,neonatal weight,postpartum hospital days and postpartum blood loss at 2 hours(P<0.05).There were no significant differences in the incidence of preterm birth,neonatal score,incidence of low birth weight or macrosomia,and incidence of postpartum hemorrhage between the two groups(P>0.05).There were statistically significant differences in postpartum 2 hours blood loss,perineal lateral resection rate and utilization rate of painless delivery between the two groups(P<0.05).There was no significant difference in the duration of second stage of labor(P>0.05).There was no significant difference in 2 hours postpartum blood loss between the two groups(P>0.05),and there was significant difference in the incidence of non-medical indications of caesarean section(P<0.05).Conclusion:(1)Most pregnant women will have fear of childbirth during pregnancy.With the aggravation of fear of childbirth,the incidence of adverse maternal and infant outcomes is gradually increasing.Enough attention should be paid to pregnant women with moderate or severe fear of childbirth during pregnancy.(2)The Internet-based group health management is centered on pregnant women,which can relieve the bad mood of single primiparas in the third trimester and improve the pregnancy outcome.
