| Objective:To compared the differences of pre-pregnancy complications,pregnancy complications,obstetric and perinatal outcomes between primipara and old primipara.Then explore the risk factors related to the adverse pregnancy outcome of the primipara.Methods : A retrospective case-control study was used to select 262 elderly primiparas who were hospitalized in the First Affiliated Hospital of Xinjiang Medical University from October 2017 to October 2018 as the study group;524 primiparas who were hospitalized in the same period were selected as the control group according to the ratio of random 1:2,and their relevant clinical data were recorded,the differences between the two groups were compared,and then multi factor Logis was performed Tic regression analysis was used to study the risk factors of adverse obstetric and perinatal outcomes.Result : 1.The incidence of chronic pelvic inflammatory disease,uterine leiomyoma,uterine scar(history of surgery),hypertension,heart disease,hypothyroidism,history of multiple abortions,etc.in the study group was significantly higher than that in the control group(P<0.05).The incidence of anemia in the control group was significantly higher than that in the study group(P<0.05).2.The incidence of pregnancy complications such as pregnancy hypertension,gestational diabetes mellitus and premature rupture of membranes in the study group was higher than that in the control group.3.Cesarean section rate: the cesarean section rate in the study group was significantly higher than that in the control group(P<0.05).4.The prognosis of perinatal infants such as NICU,neonatal asphyxia and premature birth were significantly higher in the study group than in the control group(P<0.05).Conclusion : The incidence of prenatal complications,gynecological diseases,complications during pregnancy and adverse outcomes of perinatal children is high.Therefore,the perinatal health care of the elderly primipara is the focus of perinatal medicine. |
