| Objective: In the new era,public hospitals are changing from high-speed development to high-quality development,and since 2021,the state has issued many policies to promote high-quality development of public hospitals,and it is necessary to construct a set of evaluation indexes for high-quality development in Shanghai as one of the pilot cities.As a gathering place of high-quality medical resources,tertiary public hospitals should be in the forefront in the process of high-quality development.Therefore,this study takes Shanghai’s tertiary public hospitals as the target and constructs a set of public hospital high-quality development evaluation index system with Shanghai’s characteristics to guide Shanghai’s public hospitals to achieve highquality development,and also provides some references for the study of highquality development of public hospitals by provinces and cities.Methods: This study used literature analysis,expert interviews,and interviews with knowledgeable people to initially construct the basic framework of the evaluation index system;used the Delphi method to select and modify the indexes,and finally constructed the evaluation index system for high-quality development of Shanghai’s tertiary public hospitals.The significance and operability of the indicators were calculated by Excel and SPSS25.0 statistical software to test the scientific and reliability of the Delphi method.Hierarchical analysis was used to determine the weight coefficients of indicators at each level and to conduct consistency tests.Results: The recall rate of the expert consultation forms was greater than80% in both rounds,indicating that the experts were highly motivated to consult.The degree of authority of experts was greater than 0.7,representing a good degree of authority of experts in this study.The Kendall coordination coefficients for both rounds of first,second,and third level indicators as well as overall were greater than 0.2,p≤0.05,indicating consistency in expert judgment opinions at 95% confidence intervals.Both Cronbach coefficients were greater than 0.7,and the indicators were more closely related to each other.The coefficient of variation of most of the indicators was less than 0.25,indicating that the experts’ opinions were more uniform.Finally,a complete set of evaluation index system for high-quality development of Shanghai’s tertiary public hospitals was constructed,which contains 6 primary indicators,13 secondary indicators and 34 tertiary indicators.The hierarchical analysis method was used to construct the hierarchical structure model and judgment matrix,and then the weights of each indicator were calculated,among which the highest weight of the primary indicators was the new effectiveness,and the indicators with higher weights of the secondary indicators were "medical quality" "party building leading" and "operation management".The third-level indicators with higher weights are "personnel expenditure to operational expenditure ratio" "emergency critical patient rescue success rate" and "party organization and party member team building",all of which All indicators passed the consistency test.Conclusion: The index system constructed in this study is necessary and in demand,and the reliability and scientific of the hospital quality development evaluation index system constructed by the Delphi method are good,and the feasibility of determining the index weights by the hierarchical analysis method is good,which can clearly show the key points of the hospital quality development.In the process of applying the index system,attention should also be paid to ensure the accuracy of data and simplify the evaluation operation procedures.At the same time,it is worth noting that other hospitals can refer to Shanghai’s third-grade public hospitals in the construction of the index system for high-quality development,and make appropriate modifications based on the characteristics of each hospital on the basis of this index system to build an evaluation index system suitable for them. |
