The general practitioner is a highly comprehensive medical talent,who mainly undertakes the integrated services of prevention and health care,diagnosis and treatment of common diseases and frequently occurring diseases,referral,patient rehabilitation and chronic disease management,health management,etc.at the grassroots level,and is the "gatekeeper" to safeguard citizens’ health rights and interests and reasonably control health expenditure investment Health management and sentinel role in public health emergencies play an important role.In particular,the outbreak of the COVID-19 since 2020 has put forward higher requirements for grassroots general practitioners to play a "sentinel" role in public health emergencies and scientifically and decisively deal with infectious disease outbreaks.This paper takes the public goods theory,stakeholder theory and Skinner’s reinforcement theory as the theoretical basis for discussion and analysis,takes the general practitioner team in Jiangsu Province as the research object,regards the construction and development of the team as a whole rather than two separate issues,brings the general practitioners in medical institutions of different levels and categories into the scope of the survey,conducts questionnaires and targeted discussions,and conducts evaluation and analysis,It comprehensively summarized the management status quo and relevant policies and measures for the construction and development of the general practitioners team in Jiangsu Province,and deeply explored the existing weaknesses.Through analysis,it was found that there were still some problems in the construction and development of the general practitioners team in Jiangsu Province,such as imperfect salary system,unreasonable incentive guidance,lack of restraint mechanism,and relative absence of the "gatekeeper" function.This paper goes back to the source to explore the reasons behind these problems,which are mainly caused by the lag of legal construction,the poor applicability of industry standards and evaluation systems,the imperfect compensation mechanism for public service and the weakness of their own capacity building.In view of the main reasons restricting the construction and development of general practitioners in Jiangsu Province at present,and in order to better play the role of "gatekeeper" of general practitioners,this paper proposes reasonable and feasible countermeasures and suggestions in improving top-level design,improving training and use incentive policies,strengthening the grass-roots medical and health service system,promoting family doctors’ contract service,establishing a hierarchical diagnosis and treatment system,and improving service capacity and level,With a view to accelerating the construction and development of the general practitioner team in Jiangsu Province,giving full play to the role of the general practitioner as the "gatekeeper",further highlighting the role of supporting and safeguarding the health cause,and safeguarding the health rights and interests of the people. |
