Background Currently,the number of new cases of bladder tumors is increasing year by year,and the number of patients undergoing transurethral resection for bladder tumors is also increasing at the same time.However,bladder spasm after this surgery still brings great pain to patients,and also puts forward higher requirements for clinicians to deal with it.Mirabegron,a novel β3 agonist,can effectively relieve lower urinary tract symptoms,but there is a window for the drug to reach the effective blood concentration.To explore more effective ways of administration and efficient control of postoperative bladder spasm is of great significance for reducing preoperative risk,reducing patients’ pain and improving treatment effect.Objective To recruit patients undergoing transurethral resection of bladder tumor for bladder tumor,and to study the efficacy of prophylactic use of microphone in relieving bladder spasm after transurethral resection of bladder tumor.Methods(1)Data for patients with TURBT were collected from the Second People’s Hospital of Yichang City from November 2021 to January 2023.The surgery was performed by senior physicians with more than 10 years of surgical experience in the Department of Urology of the Second People’s Hospital of Yichang City,and the patients were divided into three groups by simple random method according to the inclusion and exclusion criteria.Mirabegron prevention group: 50 mg Mirabegron sustained release tablets qd was given two days before surgery;Mirabegron treatment group: 50 mg Mirabegron sustained-release tablets qd was given after the occurrence of BS after surgery;Blank control group: No intervention was made,and symptomatic treatment with non-steroidal drugs was given when symptoms were severe.(2)compare the basic data of the three groups(gender,age,difference of hemoglobin before and after surgery,number of wounds,and diameter of wounds)to understand whether the three groups are comparable.(3)The data(frequency of bladder spasm,duration of bladder spasm,amount of bladder flushing solution used,difference of hemoglobin before and after discharge,VAS score,PPBC score and Qo R-15 score)of the three groups were compared 7 days after surgery and 1 day after catheter removal,to determine whether all indicators were statistically significant,and to evaluate the degree of relief of bladder spasm after surgery and the benefit of patients in each group.Results(1)A total of 90 patients were reflected in the present study,with 30 subjects in each group.The basic data of patients in the three observation groups(gender ratio,age,difference of hemoglobin before and after surgery,number and diameter of wounds caused by surgery)were compared in pairs,and there was no statistical significance among all groups(P≥0.05).(2)All the 90 patients included in the study successfully completed surgery.All patients completed postoperative follow-up,did not withdraw from the study,and no serious surgical complications or adverse drug reactions occurred during the postoperative observation period.The daily frequency and duration of bladder spasm in the prevention group were better than that in the treatment group,and the difference between the treatment group and the control group was statistically significant(P < 0.05).There was no significant difference in the frequency and duration of bladder spasm between the prevention group and the treatment group on the 5th to 8th day after surgery(P≥0.05),but the difference between the two groups and the control group was statistically significant(P < 0.05).VAS score of the prevention group was preferable to that of the treatment group and the control group(P < 0.05)in the 3 days after surgery.There was not any difference between the prevention group and the treatment group on the 4 to 6 days after surgery(P≥0.05),but better than that of the control group(P < 0.05).There was not any statistical difference between the three groups after the 7days after surgery(P≥0.05).VAS score of the treatment group was not significantly different from that of the control group on the first day after surgery(P≥0.05),but there was significant difference between the two groups on the second to sixth day after surgery(P < 0.05).There was no significant differences between the three groups in terms of the difference of hemoglobin at admission and delivery and the number of bladder flushing liquid bags after operation(P≥0.05).There was no significant differences in the PPBC score and Qo R-15 score among the three observation groups(P≥0.05).Conclusion The use of Mirabegron after transurethral resection of bladder tumors can effectively alleviate the occurrence of bladder spasm,and the prophylactic use of Mirabegron before surgery is more beneficial to reduce postoperative bladder spasm and effectively reduce the patient’s pain,and it is recommended to use Mirabegron prophylactically before transurethral resection of bladder tumors.
