Objective:1.Through the clinical application of targeted penetration therapy of traditional Chinese medicine to intervene bladder spasm in patients after electrosurgical resection of bladder cancer,to explore the effectiveness of targeted penetration therapy of traditional Chinese medicine to relieve bladder spasm in patients after electrosurgical resection of bladder cancer;2.To explore the significance of integrated nursing of Chinese and Western medicine in achieving the goal of accelerating the outcome of rehabilitation surgery.Methods:This is a prospective randomized controlled clinical trial.72 inpatients with bladder malignant tumor in the urological department of a Grade-III tumor hospital in a province from October 2021 to July 2022,and 72 patients who were treated with transurethral resection for bladder tumor and met the exclusion criteria,were selected as the study objects.Patients were randomly divided into experimental group and control group,with 36 cases as experimental group and 36 cases as control group.Patients in different groups were placed in different areas.Patients in both groups were given corresponding nursing care in accordance with clinical nursing guidelines after electrosurgical resection of bladder cancer.On this basis,the experimental group was also given directed TCM transdrug therapy as instructed by the doctor,25min/time,twice a day,for consecutive 6 days.The general information,frequency of bladder spasm,postoperative bladder irrigation time,catheter indent time and postoperative hospital stay of the two groups were recorded,and the difference of bladder spasm symptom score(BSSS),pain numerical score(NRS)and postoperative blood loss was observed between the two groups on the postoperative day,frist day,3rd day and 6th day.All data were processed by SPSS 26.0 statistical software.The comparison of general data and basic information of nursing staff between the two groups was conducted by T-test and chi-square test.The comparison of grade data between the two groups was conducted by Mann-Whitney U test.The score results of bladder spasm symptom,pain number score and postoperative blood loss of the two groups at different time points were statistically analyzed by repeated measure analysis of variance.The comparison of bladder irrigation time,catheter time and postoperative hospital stay of the two groups was performed by two independent sample t test,and the test statistics and corresponding p value were given,the test levelα=0.05.Results:1.In this study,72 patients were included,with a total of 4 cases shedding,and 68 effective samples were finally recovered,including 33 cases in the experimental group and 35 cases in the control group.There was no statistical significance in the comparison of general data between the two groups(P<0.05).2.Symptom score of bladder spasm:The frequency of bladder spasm in experimental group was significantly lower than that in control group after directional penetration intervention,with statistical significance(Z=-3.321,P<0.001).There were interaction effects between the two groups at different time points(Waldχ时间2s=82.10,Waldχ组间2s=3.07,Waldχ交互2s=6.41,p<0.05).Simple effect analysis showed that there was no statistical significance on the day of surgery(p>0.05).On the 1st,3rd and 5th day after surgery,the score of bladder spasm in experimental group was lower than that in control group,the difference was statistically significant(p<0.05).3.Pain numerical scoring method:There were interaction effects between the two groups at different time points and time(Waldχ时间2s=80.846,Waldχ组间2s=10.701,Waldχ交互2s=4.409,p<0.05).Simple effect analysis results showed that there was no statistical significance on the day of surgery(p>0.05).On the 1st,3rd and 5th day after surgery,the pain score of experimental group was lower than that of control group,the difference was statistically significant(p<0.05).4.Postoperative blood loss:The postoperative blood loss scores of the two groups at different time points had intergroup and time interaction effects(Waldχ时间2s=71.499,Waldχ2s=9.864,Waldχ交互2s=4.724,p<0.05).Simple effect analysis results showed that there was no statistical significance on the day of surgery(p>0.05).Postoperative blood loss scores of experimental groups were lower than those of control group at the 1st,3rd and 5th days after surgery,with statistical significance(p<0.05).5.Postoperative bladder irrigation time:Compared with the experimental group and the control group,the postoperative bladder irrigation time of the control group was longer than that of the experimental group,the difference was statistically significant(Z=-6.848,P<0.001)6.Indwelling catheter time:The indwelling catheter time of experimental group and control group was longer in the control group than in the experimental group,and the difference was statistically significant(Z=-6.239,P<0.001).7.Postoperative hospital stay:Compared with the experimental group and the control group,the postoperative hospital stay of the control group was longer than that of the experimental group,the difference was statistically significant(Z=-6.018,P<0.001).Conclusion:1.Targeted penetration therapy of traditional Chinese medicine can significantly reduce the frequency of bladder spasm in patients after electrosurgical resection of bladder cancer and relieve the degree of symptoms of bladder spasm,which is recommended as a clinical method of traditional Chinese medicine nursing intervention for bladder spasm in patients after electrosurgical resection of bladder cancer.2.Targeted penetration therapy of traditional Chinese medicine can significantly reduce the degree of pain and postoperative blood loss of patients after electrosurgical resection of bladder cancer.3.Targeted penetration therapy of traditional Chinese medicine can significantly reduce the bladder irrigation time,catheter indwelling time and postoperative hospital stay of patients after electrosurgical resection of bladder cancer,effectively realizing the accelerated postoperative recovery of patients.
