Objective:To investigate the safety and feasibility of bipolar electrocoagulation forceps in open thyroid cancer operation.Methods:Retrospective analysis was performed on the clinical data of 140 patients with papillary thyroid cancer who received open total thyroidectomy and bilateral neck regionⅥlymph node dissection in the First Affiliated Hospital of Gannan Medical University from November 2019 to September 2022.According to the different energy surgical instruments used in the operation,140 patients were divided into ultrasonic knife group and bipolar electrocoagulation forceps group,with 70 cases in each group.The operation time,intraoperative blood loss,postoperative drainage,hospital stay,number of lymph node dissection,changes in serum parathyroid hormone(PTH),Ca2+levels and postoperative complications were compared between the two groups.Results:1.There was no significant difference in operation time(P=0.140)and intraoperative blood loss(P=0.786)between the bipolar electrocoagulation forceps group and the ultrasonic knife group.2.The hospital stay and postoperative drainage in the bipolar electrocoag-ulation forceps group were significantly lower than those in the ultrasonic knife group(all P<0.05).3.The number of lymph node dissection in the bipolar electrocoagulation forceps group was higher than that in the ultrasonic knife group,and the difference was statistically significant(P=0.008).4.The overall analysis showed that there were statistically significant differences in the two groups on the 1,3,7 and 30 days after surgery of serum PTH and Ca2+levels between groups,time point comparison and interaction(all P<0.05).Further pairwise comparison,intra-group comparison:the serum PTH and Ca2+levels of the two groups at each time point after operation were significantly lower than those before operation(all P<0.05).Comparison between groups:the serum PTH and Ca2+levels in the bipolar electrocoagulation forceps were higher than those in the ultrasonic knife group at each time point after operation,and the difference was statistically significantat(all P<0.05).5.The total incidence of postoperative complications in the bipolar electr-ocoagulation forceps group was lower than that in the ultrasonic knife group,and the difference was statistically significant(P=0.020).Conclusion:Bipolar electrocoagulation forceps is a safe surgical instrument,which has more advantages than ultrasonic knife in the fine operation of dissecting thyroid and its related tissues,dealing with the safety of recurrent laryngeal nerve and peri-parathyroid small vessels,and reducing operation-related complications.
