| Aims 1?To collect general clinical data of all endoscopic thyroid surgery patients from 2013 to 2017,briefly summarized the types,quantity,age,gender and operation time,and preliminarily understood the clinical application of endoscopic thyroidectomy in our hospital.2?Analyzed the characteristics of 6 cases of endoscopic thyroidectomy through oral vestibule in our hospital.Analyzed the Safety and effectiveness.Discuss its clinical application value.Combined with the published literature at home and abroad to analyze its clinical application status and prospects.Methods: 1?From January 2013 to December 2017,all general data of endoscopic thyroid surgery were collected from the medical records room of our hospital,including type of operation,age,sex,operative time,pathological diagnosis,complications and so on.2?Research object: From May 2017 to november 2017,we selected 6 patients with benign thyroid diseases who were admitted to the department of mammary division of our hospital,they had operations via oral vestibule Endoscopic thyroidectomy,retrospective analysis of its clinical characteristics,observation and summary of patients' operation and postoperative recovery,at the same time,searched the relevant documents,summary and analysis of clinical application of endoscopic thyroid surgery via oral vestibule.Results 1 ? From January 2013 to December 2017,a total of 153 patients underwent endoscopic thyroidectomy,including 124 cases of minimally invasive video assisted thyroidectomy,3 cases of endoscopic thyroid surgery via submaxillary pathway,20 cases of breast approach endoscopic thyroidectomy.6 patients underwent endoscopic thyroid surgery through oral vestibule.138 cases were female and 15 cases were male.The pathological findings were nodular goiter,thyroid micropapillary carcinoma,thyroid adenoma,Hashimoto's thyroiditis,postoperative complications: recurrent laryngeal nerve injury,infection,hypocalcemia,subcutaneous effusion,precervical ecchymosis.2?Endoscopic thyroidectomy via oral vestibule was performed in 6 cases in our hospital.Are all women.Average age of 41.7 years,The average operative time was 176 minutes.The average postoperative hospital stay was 5.5 days.The average intraoperative bleeding volume of 27.5ml,The average postoperative drainage 20.8ml,No transmissible open surgery was performed during the operation.All patients had pain and discomfort after the operation.3 days after the operation,there was a gradual remission.One patient developed post-operative cervical ecchymosis,asymptomatic hypocalcemia.The rest of the patients had no obvious discomfort after operation.The result of operation is satisfactory.Postoperative follow-up was 5-8 months.No special discomfort,satisfactory cosmetic effect,At present,there are more than 20 reports on the clinical application of transoral endoscopic thyroidectomy at home and abroad.Except for a few cases of infection,transient recurrent laryngeal nerve paralysis,asymptomatic hypocalcemia,transient hypothyroidism,mental nerve injury,precervical ecchymosis,no major surgical complications occurred.Conclusion: 1? Conventional multiplex endoscopic thyroidectomy included minimally invasive video assisted thyroidectomy,breast approach endoscopic thyroidectomy,endoscopic thyroid surgery with submaxillary pathway,and transoral endoscopic thyroidectomy vestibular approach.Individualized operation can be selected according to the characteristics of the disease and the wishes of the patient.2?Except 1 patient with anterior cervical ecchymosis,asymptomatic hypocalcemia were found in 6 patients in our hospital.No major surgical complications occurred.The beauty effect is remarkable.It is proved that endoscopic thyroidectomy via oral vestibule is safe and feasible.Combined with the analysis of the clinical application of endoscopic thyroidectomy through oral vestibule at home and abroad,the operation is safe and effective.TOETVA can be widely used by experienced thyroid surgeons in selected patients. |
