Objective:To monitor the preventive effect of 0.02% low concentration atropine eye drops on myopia through multispectral refraction topography(MRT).Methods: Part Ⅰ,study of retinal defocus values of peripheral retina in pre-myopia:Select 337 patients(337 eyes)with pre-myopia in the First Affiliated Hospital of Dali University and Dali Ophthalmology hospital for cross-sectional study to improve uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA),spherical equivalent(SE),axial length(AL),intraocular pressure(IOP),mean keratometer data(Km),Multispectral refractive topography(MRT)and fundus examination under slit lamp,and observe the distributing characteristics of the retinal defocus values in different field of vision(total retinal defocus values TRDV,retinal defocus values of the15° retina RDV-15°,retinal defocus values of the 30° retina RDV-30°,retinal defocus values of the 45° retina RDV-45°,retinal defocus values of peripheral retina RDV-(53°-15°),retinal defocus values of the superior retina RDV-S,retinal defocus values of the inferior retina RDV-I,retinal defocus values of the temporal side retina RDV-T,retinal defocus values of the nasal side retina RDV-N),and then analyze relevant factors.Part Ⅱ,study on the treatment of low concentration atropine eye drops monitored by multispectral refractive topography: 110 patients(110 eyes)were prospectively selected from the above patients for follow-up study,of which 60 patients(60 eyes)were taken as atropine group,and 0.02% low concentration atropine eye drops were used once before going to bed every other day,and follow up the above examination results after nine months;50 cases(50 eyes)served as the control group,without using any eye drops,also recorded as the follow-up results after nine months.Compared the two groups with the spherical equivalent,axial length,retinal defocus values of peripheral retina and other biological parameters of the eye.Results:Part Ⅰ:1.Based on the retinal defocus values of peripheral retinal measured by multispectral refractive topography,it was divided into four types,in which volcanic mouth type accounts for 4.15%(14/337),saddle type accounts for 9.79%(33/337),hemiolateral upturned type accounts for 64.69%(218/337),and relatively flat type accounts for 21.36%(72/337).2.In the ring partition,RDV-45°>RDV-30°>RDV-15°,and the amount of retinal defocus values of RDV-45° was the largest,it was 0.13(-0.07,0.30)D.3.The retinal defocus values of peripheral retinal was shown in the quadrants: RDV-I>RDV-N>RDV-T>RDV-S,where the superior retinal was myopic defocus,which reached-0.43(-0.81,-0.03)D,and the other three quadrants were hyperopic defocus,and the hyperopia defocus in the inferior retinal is the most obvious,which reached 0.51(0.16,0.82)D.Our study investigated the superior and the inferior,temporal side and nasal side were asymmetry.4.The Spearman correlation analysis showed that retinal defocus values of peripheral retina was negatively correlated with the spherical equivalent(p<0.05);It was positively correlated with age and axial length(p<0.05);There was no correlation with gender,IOP and mean keratometer data(P>0.05).Part Ⅱ:1.There was no statistical difference between the atropine group and the control group at baseline in terms of spherical equivalent,axial length,intraocular pressure,mean keratometer data,retinal defocus values of peripheral retinal(RDV-15°,RDV-30°,RDV-45°,RDV-(53°-15°),RDV-S,RDV-I,RDV-T,RDV-N)(P>0.05).2.Compared with the follow-up data of the control group and atropine group,the spherical equivalent,TRDV,RDV-(53°-15°),RDV-30°,RDV-45°,RDV-S and RDV-I in atropine group was less than that in the control group(P<0.05);There was no difference in axial length,intraocular pressure,mean keratometer data,RDV-15°,RDV-T and RDV-N in atropine group and the control group(P>0.05).3.Compared with the atropine group before and after treatment,the difference of spherical equivalent and axial length was significant(P<0.05),while the difference of intraocular pressure,mean keratometer data,TRDV,RDV-(53°-15°),RDV-15°,RDV-30°,RDV-45°,RDV-S,RDV-I,RDV-T and RDV-N was not significant(P>0.05).4.Compared with the control group before and after treatment,the difference of spherical equivalent,axial length,TRDV,RDV-(53°-15°),RDV-45°,RDV-S and RDV-T was significant(P<0.05).The difference of intraocular pressure,mean keratometer data,RDV-15°,RDV-30°,RDV-I and RDV-N was not significant(P>0.05).Conclusions:1.The retinal defocus values of peripheral retina in pre-myopia is mild hyperopia defocus,indicating that the risk of myopia was relatively high.2.0.02% low concentration atropine eye drops can cause peripheral relative myopic defocus by multispectral refraction topography,which was consistent with the reduction of equivalent spherical diopter and the reduction of myopia. |
