Research ObjectThis study aims to evaluate the efficacy of repeated transcranial magnetic stimulation combined with MOTOmed intelligent exercise training in the recovery of lower limb dysfunction in patients with ischemic stroke,so as to better apply it in clinical practice and serve patients.Research MethodologyIn this study,75 patients with ischemic stroke who were treated in the Department of Rehabilitation Medicine of our hospital were selected as the subjects.The patients were divided into 3 groups according to the randomization principle,namely group A r TMS,group B MOTOmed intelligent exercise training.Group C r TMS combined with MOTOmed intelligent motor training.Patients in 3 groups received routine rehabilitation therapy including good limb placement,exercise therapy,joint loosening training,low-frequency pulse electrotherapy and ordinary acupuncture during the whole study.Fugl-Meyer assessment(FMA)score was used to evaluate the lower limb motor function,Berg Balance Scale(BBS)was used to evaluate the balance ability of patients,and Modified Barthel index(MBI)was used to evaluate the ability of daily living of patients Analysis;The electrophysiological indexes of three groups were evaluated and analyzed by MEP latency,MEP amplitude and central motor conduction time(CMCT)were used to monitor the changes of neuroelectrophysiological indexes in three groups.The indexes before and after treatment were evaluated by physicians who did not participate in the study and did not know the group。Research Results1.Before treatment,there were no significant differences in FMA score,BBS score,MBI score,MEP latency,MEP amplitude and CMCT among the three groups(P>0.05).2.Comparison within the three groups after treatment,FMA score,BBS score and MBI score of the three groups after 4 weeks of treatment were higher than those before treatment,and the differences were statistically significant(P<0.05),the MEP latency and CMCT in the three groups decreased compared with before treatment,and the MEP amplitude was prolonged compared with before treatment,the differences were statistically significant(P<0.05).3.After 4 weeks of treatment,FMA score,BBS score and MBI score of combined group were significantly higher than those of r TMS group and MOTOmed group,and the differences were statistically significant(P<0.05),MEP latency and CMCT in combined group were lower than those in r TMS group and MOTOmed group,and MEP amplitude in combined group was higher than that in r TMS group and MOTOmed group,the differences were statistically significan(P<0.05),but there were no statistically significant differences in FMA score,BBS score,MBI score,MEP latency,MEP amplitude and CMCT between r TMS group and MOTOmed group after treatment(P>0.05).Research ConclusionrTMS combined with MOTOmed intelligent exercise training embodies the concept of“centrai-peripheral-central”closed-loop rehabilitation therapy,and the combined treatment of the two has better efficacy,and its therapeutic effect is superior to r TMS and MOTOmed intelligent exercise training alone,which can significantly improve the motor dysfunction of lower limbs,improve the ability of daily living.The combined treatment plan is worthy of clinical promotion. |
