Objective:To explore the clinical effect of acupuncture combined with blood-letting therapy on menstrual headache of Yin deficiency and Yang excess. The visual analogue scale(VAS),headache symptom rating scale, headache impact measurement and TCM syndrome rating scale were used to observe the changes in the condition of each course of treatment. The long-term treatment effect was determined by follow-up.To verify the effectiveness of this therapy in the treatment of menstrual headache of Yin deficiency and Yang excess,so as to provide a new treatment method for menstrual headache of Yin deficiency and Yang excess in clinical practice. Methods: In this study,63 patients with migraine of Yin deficiency and Yang excess type who met the diagnostic criteria and inclusion criteria were selected as the research objects. The treatment group and the control group were set up for comparative study, and the patients were assigned by random number table method.A total of 32 patients were assigned to the treatment group and treated with ordinary acupuncture combined with bloodletting.A total of 31 patients were assigned to the control group and treated with ordinary acupuncture.During the treatment,2 cases dropped off in the treatment group and 1 case dropped off in the control group. The treatment was started 1 week before menstruation and was given once daily until the end of menstruation. The blood prick was started on the day of headache onset and was given every other day for 3 times per menstrual cycle. Both groups were treated for 3 menstrual cycles. One menstrual cycle was defined as one treatment course, and after the completion of three treatment courses,a return visit was performed in the next menstrual cycle. The pain degree evaluation, headache symptom rating scale, headache impact measurement evaluation and TCM syndrome evaluation were compared between the two groups before treatment, after 1 course of treatment,2 courses of treatment,3 courses of treatment and return visit to determine the treatment effect of the two groups. Results:By comparing the clinical efficacy of the two groups,4 cases were cured in the treatment group, accounting for 13.33% of the total.Eighteen cases(60.00%)were markedly effective.Seven cases(23.33%)were effective. One case was ineffective, accounting for 3.33% of the total; The total effective rate was 96.67%.In the control group,2 cases were cured, accounting for 6.67% of the total;Seven cases(23.33%)were markedly effective.Sixteen cases were effective, accounting for 53.33% of the total;Five cases were ineffective, accounting for 16.67% of the total; The total effective rate was 83.33%.The clinical efficacy of the two groups was statistically significant, indicating that the clinical efficacy of the treatment group was different from that of the control group, and the clinical efficacy of the treatment group was better than that of the control group. In the treatment group,3 cases were cured, accounting for 10.00% of the total;15 cases(50.00%)were markedly effective.Seven cases(23.33%)were effective.Five cases were ineffective, accounting for 16.66% of the total; The total effective rate was 83.33%.In the control group,1 case was cured, accounting for 3.33% of the total;Eight cases(26.67%)were markedly effective.14 cases(46.67%)were effective.Seven cases were ineffective, accounting for 23.33% of the total; The total effective rate was 76.67%.The clinical efficacy of the two groups was statistically significant, indicating that the improvement of TCM syndrome in the treatment group was better than that in the control group. Conclusion:Both ordinary acupuncture and acupuncture combined with blood-letting therapy have therapeutic effect on menstrual headache of Yin deficiency and Yang excess type. The clinical effect of acupuncture combined with blood-acupuncture on menstrual headache of Yin deficiency and Yang excess is better than that of ordinary acupuncture.
