Retrospective Analysis Of TCM Syndrome And Medication Rule In Senile Type 2 Diabetes Mellitus

Objective: Multiple national diabetes epidemiological surveys show that the number of elderly patients with type 2 diabetes is increasing year by year and has become themainstream population of diabetes.This study will explore the distribution of TCM syndromes and medication in elderly patients with type 2 diabetes,and explore the law,so as to provide help for the TCM treatment of type 2 diabetes in elderly patients.Methods: Taking a retrospective study approach,A total of 300 type 2 diabetes patients aged 60 years old in the Department of Geriatrics from December 2018 to December 2021 at the Affiliated Hospital of Changchun University of Traditional Chinese Medicine were selected as the research subjects.The patient’s basic information,four diagnosis data,laboratory test results,traditional Chinese medicine prescription and other related information were recorded,which were recorded in Microsoft Excel for storage,and the original information database was established.SPSS 26.0,IBM SPSS Modeler 18.0software,factor analysis,association analysis and other methods were used to conduct a statistical analysis of syndrome distribution and medication rule in elderly patients with type2 diabetes.Results:1.Basic features: A total of 300 patients met the criteria,with an age distribution between 60 and 93 years,with a mean age of 68.5 years.Among them,there were 147 male patients and 153 female patients,and the male to female ratio was: 0.96:1.2.Distribution of syndrome:After frequency analysis of symptomstongue coating and pulse image in 300 cases,32 high frequency information was obtained.The four-diagnosis information was introduced into SPSS 26.0 for factor analysis,and seven different syndrome types of the elderly type 2 diabetes patients were obtained,respectively:liver-kidney Yin deficiency syndrome,qi-yin deficiency syndrome,syndrome of phlegm-heat(dampness)interlocking,syndrome of heat causing injury to Jin,syndrome of Qi and Yin deficiency with blood stasis,syndrome of deficiency of Yin and Yang,syndrome of liver and kidney Yin deficiency and blood stasis.The main types of syndromes are similar to Qi and Yin deficiency and similar to liver and kidney Yin deficiency.3.Drug use law: Through the frequency analysis of drugs,the high frequency drugs were astragalus membranaceus,poria cocos,Radix rehmanniae,licorice,ophiopogon,angelica,bupleurum,pueraria and other 42 flavors.There are three kinds of high frequency drugs: tonic medicine,antipyretic medicine,blood activating and stasis removing drugs.The medicinal properties were mainly cold,warm and flat.Sweet,bitter and sinuous were the main medicinal tastes.Drugs mainly belong to the spleen,kidney and liver channels.After association rule analysis,31 correlation conditions were obtained among different drugs,and the top three were schisandra chinensis and ophiopogon,peach kernel and safflower,moutan bark and poria cocos.The top three confidence percentages were safflower,angelica and peach kernel,moutan bark,dogwood and yam,peach kernel,poria cocos and safflower.After cluster analysis,a total of 7 core combinations were extracted:(1)peach kernel,safflower,Ligusticum chuanensis,angelica sinensis,(2)cassia twig,spatholoba japonica,(3)Corni officinalis,yam,peony bark,poria cocos,(4)Radix tangerini,pinellia pinellia,Magnolia officinalis,(5)bupleurum,Scutellaria,(6)Rhizome,trichosanthin,dark plum,Kudu root,(7)scrophulariae,jade bamboo,ophiopogonis ophiopogonis,schisandra chinensis,codonopsis codonopsis.Conclusion: Elderly type 2 diabetes mainly in the spleen(stomach),liver,kidney.The most common syndromes are Qi and Yin deficiency syndrome and liver and kidney Yin deficiency syndrome.It is suggested that Yin deficiency or Qi deficiency is the basis of senile type 2 diabetes,phlegm turbidness and blood stasis is the standard,and many deficiency and solid are mixed.The treatment should treat both symptoms and root causes,and those who treat deficiency by treating symptoms should nourish Qi and blood,nourish Yin and replenish Yang and moisten dryness.The real person should be Qi phlegm,Yu Qing stomach,blood circulation and collaterals.
