A Meta-analysis Of The Method Of Nourishing Yin And Submerging Yang And Benefiting The Kidney And Consolidating Essence In The Treatment Of Renal Damage In Hypertension

Objective: The clinical manifestations of hypertensive kidney damage are atypical,and Western medicine has limited treatment options,while Chinese medicine has achieved better clinical efficacy in treating this disease by nourishing yin and concealing yang and benefiting the kidney and consolidating essence.In this study,we searched the literature from several databases,and used the inclusion and exclusion criteria to select the literature for the study,and summarized the data from the included literature for Meta-analysis to demonstrate the effectiveness and safety of the method of "nourishing yin and concealing yang,benefiting the kidney and consolidating essence" in the treatment of hypertensive kidney damage.Methods: Several domestic and foreign databases were searched by computer,and the search was limited to the period from the establishment of the database to June 2022.Two researchers filtered the literature that met the study requirements based on the inclusion and exclusion criteria,and applied Rev Man 5.4 software to conduct bias-dependent risk assessment and Meta-analysis of the data.The prescriptions of the Chinese medicines included in the clinical trials were extracted,and the prescriptions and names of the Chinese medicines were standardized and entered into Microsoft Excel 2020,and the frequency of use of the Chinese medicines and the taste of the medicines were counted,and the association rule analysis and drug clustering analysis were performed using statistical software to derive the medication patterns.Results:A total of 17 papers were included,with a total sample size of 1551 cases,including 836 cases in the experimental group and 715 cases in the control group.Compared with the treatment with western medicine,the treatment of hypertensive kidney damage with Chinese herbal soup significantly improved the clinical symptoms(P<0.00001),reduced SBP(P=0.005),DBP(P=0.0006),urinary microalbumin(P=0.008),urinary β-2 microglobulin(P=0.007),andserum cystatin C(P=0.0006).inhibin C(P=0.003),blood creatinine(P=0.0002),urea nitrogen(P<0.0001)urinary NAG enzyme(P=0.02)The differences were statistically significant.Analysis of the prescriptions of Chinese herbal medicines in 17 articles showed that the drugs with higher frequency were Cornus officinalis,Poria,Chuanxiu Knee,Salvia,Crocus,and Shu Di Huang.The medicinal properties were mostly cold and flat drugs,and the medicinal flavors were mostly bitter and sweet.The highest support for second-order association according to the association rule analysis was Poria-Poria-Zeigong,and the highest support for third-order association was Cornus-Poria-Zeigong.Conclusion: The results of the current study showed that compared with the simple application of basic Western medicine treatment,the method of nourishing yin and submerging yang and benefiting kidney and consolidating essence in the treatment of hypertensive kidney damage not only significantly improved clinical symptoms,but also improved outcome indicators such as SBP,DBP,urinary microalbumin,urinary β-2microglobulin,serum cystatin C,blood creatinine,urea nitrogen,and urinary NAG enzyme.The dosing pattern of the method of nourishing Yin and submerging Yang and benefiting the kidney and consolidating essence in the treatment of hypertensive kidney damage was analyzed.
