Based On Data Mining Technology To Explore Professor Zhang Shoulin’s Academic Thought Of Treating Stage 3-5 Chronic Kidney Disease From Spleen And Stomach

Objective: Through the ancient and modern medical record cloud platform,using the method of data mining,the clinical medication characteristics and compatibility rules of traditional Chinese medicine(TCM)of professor Zhang in the treatment of chronic kidney disease(CKD)stage 3-5 from spleen and stomach were explored,and his academic thoughts were further explored,in order to provide new theoretical guidance and medication ideas for clinical practice.Methods: 1.By collecting the medical records of Professor Zhang during the outpatient period from September 2020 to October 2022,the general data and relevant information of the patients were entered into the database.2.The case data of the database were screened,the case data that met the requirements were extracted,and the relevant data information in the self-built database was statistically analyzed by the ancient and modern medical record cloud platform.3.Summarize Professor Zhang’s TCM clinical medication characteristics and compatibility rules in the treatment of chronic kidney disease stage 3-5 from spleen and stomach,and then discuss his academic thoughts.Results:1.A total of 185 patients were enrolled in this study,including 111 male patients and 74 female patients.The age of onset ranged from 24 to 83 years old,and the vast majority of patients were concentrated between 50 and 59 years old.In staging statistics,most patients were in CKD3 stage.The most common clinical symptoms were fatigue,nausea,nocturia,dry mouth,lower limb edema,loose stool,chills and other spleen-stomach related symptoms.In the statistical analysis of tongue and pulse,most of them were pale red tongue with white fur and heavy and weak pulse.2.In this study,a total of 426 TCM prescriptions(including the first and subsequent visits)were used,and 263 different TCM prescriptions were used.The frequency statistics showed that the core drugs in this study included: Codonopsis radix,Radix pseudorrhoeae,Tangerine peel,Rhubarb,Astragalus membranaceus,Pinellia pinellia,fried malt,Atractylodes rhizoma atractylodes,Hawthorn,Tuporia,bamboo mushroom,jade bamboo,yam,herb herb,Angelica Dahurica,Loquat leaf,Jiaoliu Shenshu,Perilla leaf,Angelica,Bixie xie,reed root,white cardamom,Rhizoma coptidis,grass kernels,Jinneijinx,dandelion,water red flower seed,honeysucker vine,Agastache,Jixuecao.In the statistics of the four qi and five tastes of medicines,"warm,flat,cold","sweet,bitter,and spicy" were the main ones.3.Through the analysis of association rules,it was found that the common medication pairs of professor Zhang were: Pinellia-rhubarb,Radix notoginseng-Radix codonopsis,Radix notoginseng-Radix astragali,Radix astragali-Radix Codonopsis,tangeri-Radix notoginseng,Tangeri-pinellia.Cluster analysis showed that the common drug combinations were as follows:(1)Codonopsis radix and Pseudostellariae radix;(2)Astragalus mongholicus,loquat leaves,tangerine peel,fried malt,crane grass;(3)Rhubarb and Pinellia;(4)Atractylodes atractylodes and hawthorn;(5)Angelica dahurica,bamboo shavings and jade bamboo;(6)Yam,Tuporia,six divine operas.Through complex network analysis,the core prescription was obtained as follows: Codonopsis radix,Radix pseudotumor,Tangerine peel,Astragalus membranaceus,Rhubarb,Pinellia pinellia,Atractylodes rhizoma,fried malt,yam,bamboo,angelica dahurica.Conclusion:1.Based on the analysis of drug meridians,it can be concluded that the most frequently used drugs are the drugs of spleen and stomach,reflecting professor Zhang’s idea of treating CKD stage 3-5 from the perspective of spleen and stomach.2.Through the analysis of high-frequency drugs,common drug pair and core prescription,Professor Zhang believes that this disease is the pathogenesis of the syndrome of deficiency of the essence and excess of the symptoms.The deficiency of the essence is the deficiency of spleen and kidney,and the essence is the internal content of dampness,blood stasis and poison.3.Treating chronic kidney disease from spleen and stomach can be applied to the whole process of treating chronic kidney disease.Treating kidney from spleen and stomach and treating spleen and kidney together can provide a new drug idea for treating stage 3-5 chronic kidney disease in clinic.
