Research objectiveThis paper used data mining related technologies to conduct a comprehensive and objective statistical analysis of Professor Zhang Ning’s medication and prescription compatibility rules for proteinuria in chronic kidney disease,summarizing the clinical experience and academic thoughts of Professor Zhang Ning in the diagnosis and treatment of proteinuria in chronic kidney diseaselike tonifying,clearing heat and eliminating in order to lay foundation for the inheritance of her academic thoughts,standardize traditional Chinese medicine therapy of proteinuria in chronic kidney diseaseand provide data and theoretical support to directing clinical practice.Research methodsData mining technology was used to collect the medical records of renal proteinuria patients who visited Zhang Ning from January 1,2018 to December 31,2021 in compliance with screening criteria were finally included and their intact records were established into an Excel database.Then,the Ancient and Modern Medical Record Cloud Platform,IBM SPSS 26.0 and IBM SPSS Modeler 18.0 were used to conduct frequency statistics,four natures and five flavors analysis,association rules,system clustering and complex network analysis of included prescription data to conclude professor Zhang Ning’s medicinal feature,compound and compatibility in clinical treatment of proteinuria in chronic kidney disease.Research results1.1 The general dataThis study included 218 patients,among them,140 cases have definite primary disease,and 297 prescriptions with proteinuria in chronic kidney disease,of which 126(57.8%)were male and 92(42.2%)were female.The male to female ratio was 1.37.The youngest patient was 15 years old while the oldest was 91.The patients were mainly distributed in 40 to 79 years old.1.2 The frequency analysis of chinese medicineAmount to 297 prescriptions contained 165 traditional Chinese medicines included in this study.A total of 24 traditional Chinese medicines with a frequency of more than 60 occurrences.The order of chinese medicine frequency from high to low is astragalus root,bupleurum,earthworm,raw coix seed,poria,pyrrosia,rehmanniae,plantain,salvia miltiorrhiza,white peony root etc.Among them,the frequency of use of Astragalus root has the highest frequency,and the number of times is 259 times.1.3 The properties analysis of chinese medicineIn terms of four properties,the frequency of cold medicine,warm medicine,flat medicine and cool medicine were 2002 times,1165 times,834 times and 355 times,febrile medicine were used the least,only 28 times.In terms of five flavors,most of them were sweet,bitter and spicy.The sweet was 2684 times,the bitter was 1950 times,the spicy was 1051 times,the light was 489 times,the sour was 423 times and the salty was 323 times.1.4 The analysis of meridian tropismAmong the 165 traditional Chinese medicines belonging to the liver meridian were the most(2241 times).Other medicine were applied which based on frequency from most to least is spleen meridian(1972 times),lung meridian(1856 times),kidney meridian(1301 times),stomach meridian(1060 times),heart meridian(956 times),bladder meridian(611 times),gallbladder meridian(415 times),small intestine meridian(312 times),large intestine meridian(258 times),pericardium meridian(131 times)and trifoci meridian(74 times).1.5 The analysis of the pharmaceutical doseAccording to statistical analysis,the conventional dose of medication is astragalus root 30g,bupleurum 10g-15g,earthworm 15g,raw coix seed 20g-30g,poria 30g,pyrrosia 15g,rehmanniae 20g,plantain 20g-30g,salvia miltiorrhiza 15g,white peony root 15g,Angelica 10g-15g,fried jujube 20g-30g,wine-treated rhubarb 6g,hawthorn 15g,dipsacus asper 15g,mulberry peel 30g,white square root 15g,rhizoma cyperi 10g,rhizoma anemarrhenae 10g,cortex phellodendron 10g,polyporus umbellatus 15g,rhizoma gastrodiae 15g,radix pseudostellariae 20g,rhizoma atractylodis macrocephalae 20g-30g.1.6 The analysis of the pharmaceutical flavorIn this 297th,the number of tastes is mainly concentrated in 15-17 flavors,of which 16 flavors are mostly used.The prescription maximum 20 flavor,minimum 8 flavor.1.7 The analysis of the association rulesAccording to association rule analysis,seven medicine pairs,namely astragalus root with earthworm,astragalus root with poria,astragalus root with salvia miltiorrhiza,astragalus root with pyrrosia and earthworm,astragalus root with raw coix seed and earthworm,astragalus root with earthworm and bupelum,astragalus root with raw coix seed and bupelum,had high confidence and support degree.1.8 The analysis of system clusterAfter systematic cluster analysis,24 medicine with frequency ≥60 times were divided into 4 groups and obtained 4 core medicine combination.Group 1 were cortex phellodendri,rhizoma anemarrhenae,cogongrass rhizome,white mulberry.Group 2 were radix rehmanniae,cornel,earthworm,pyrrosia leaf,coix seed,astragalus root,angelica sinensis and radix dipsaci.Group 3 were bupleurum,rhizoma cyperi,radix paeoniae alba,fried zizyphus jujube kernel and gastrodia elata.Group 4 were salvia miltiorrhiza,wine rhubarb,radix pseudostellariae,Poria cocos,poria poria,Plantain plantain and Atractylodes macrocephala.1.9 The analysis of complex networkAfter a complex network analysis,it is concluded that the core prescription,composition for astragalus root,cortex phellodendri,rhizoma cyperi,wine rhubarb,plantain,poria cocos,yam rhizome,radix dipsaci,pyrrosia leaf,rhizoma anemarrhenae,cogongrass rhizome,radix paeoniae alba,coix seed,atractylodes,radix rehmanniae,grifola,fried zizyphus jujube kernel,white mulberry,bupleuri,angelica sinensis,cornel,edible tulip,pseudostellaria root,rhizoma gastrodiae,earthworm and danshen root.1.10 Comparative analysis of different degrees of protein urine drug frequencyCommon drugs for light,medium and severe proteinuria were analyzed,and the frequency of using astragalus root in three stages was highest.In addition,the drugs often used in three stages include the bupleurum,earthworm,raw coix seed,fabric yellow,stone weavi,salvia,medlar,plantain,angelica.It can be summarized as a tonic medicine,blood circulation and phlegm,ductil,clearing medicine.During mild protein urine,Professor Zhang Ning commonly used hawthorn,continued;moderate protein urine stage,raw coix seed,plantain,porenter,and pyrrosia lingua’s usage frequency has increased in the previous stage,and uses wine rhubarb The product is discharged;in the severe protein urine stage,the proportion of astragalus root is 94.87%,the frequency of tuckahoe,the use of the plantain is significantly increased,and it is often seen in piglets,alisma and winter melon skin.Research conclusionProfessor Zhang Ning thinks that the pathological nature of proteinuria in chronic kidney diseaseis asthenia in origin and asthenia in superficiality which mainly with spleen and kidney deficiency.The asthenia in superficiality mainly contains damp and hot pathogens,qi stasis,blood stasis and heat toxin,involving the viscera mainly lies in the liver,spleen,lung and kidney.The treatment based on the pathogenesis is tonifying method,clearing heat method and eliminating method.The tonifying method containes invigorating spleen and supplementing qi,tonifying kidney to nourish spleen.The clearing heat method containes clearing heat and expelling damp,clearing heat and toxic materials.The eliminating method containes soothing the liver to promote the circulation of qi,promoting blood circulation to remove blood stasis.Professor Zhang Ning emphasizes on the distinguish between asthenia in origin and asthenia in superficiality,which can direct the treatment.According to the statistical analysis,it is concluded that cold medicine,warm and cold medicine and ordinary medicine are the most used in professor Zhang Ning’s treatment of proteinuria in chronic kidney disease,most of the herbs are sweet,bitter and spicy,medicine are mostly attributed to the liver,spleen,lung and kidney channels.And the medication is mild without obvious bias. |
