Clinical Observation Of Chaihu Shugan Powder For Treating Gallbladder Polyp(Liver Stagnation And Qi Stagnation Type)

Objective:To observe and evaluate the clinical efficacy and safety of Chaihu Shugan Powder in the treatment of gallbladder polyps(syndrome of liver depression and qi stagnation),and to provide evidence for clinical promotion.Method:A total of 60 patients meeting the inclusion criteria were from the hepatobiliary outpatient Department of the First People’s Hospital of Yunnan Province and the Hepatobiliary Outpatient Department of Yunnan Hospital of Traditional Chinese Medicine from October 2020 to December 2022,respectively.In this experimental study,randomized control method was adopted,patients were numbered according to the order of treatment,and random number table method was used to randomly divide the patients into two groups.One group was the treatment group,and the other group was the control group.30 patients in the treatment group were given bupleurum Shugan powder plus or minus(bupleurum 15g,Ligusticum Chuanxiong 10g,Fructus odoratum 10g,Tangerine peel 9g,peony 9g,Fructus aurantii 9g,Turmeri 10g,Coix 12g,rigidus mori 10g,Sanling 10g,Zedoary turmeric 10g,licorice 6g)to make decoction.30 patients in the control group were given Daning tablets(Shanghai Hehuang Pharmaceutical Factory),batch number:Z10910040,specifications:0.36g/tablet,3 tablets each time,three times a day,warm after meals,4 weeks as a course of treatment,continuous drug observation for 3 courses,at the end of each course,respectively before and after the treatment,imaging efficacy,TCM syndrome total score and safety index detection.Results:1.Before treatment,there were no significant differences in gender(P=0.598>0.05),age(P=0.808>0.05),total score of TCM syndrome(P=0.140>0.05)and maximum diameter of polyp(P=0.065>0.05)between the two groups,indicating that the statistical data of the two groups were comparable.2.Imaging efficacy:Overall efficacy:Before treatment,the mean diameter of polyps in the treatment group was 4.54mm,and at the 4th,8th and 12th week of treatment,the diameter of polyps in the treatment group was 3.17mm,2.78mm and 2.55mm,respectively.The paired T-test was used in the treatment group,and statistical analysis showed that P=0.045,0.023 and 0.017.The difference was statistically significant(P<0.05).The average diameter of polyps in the control group was 4.11mm.After the 4th,8th and 12th weeks of treatment,the average diameter of polyps was 3.45mm,3.68mm and 3.29mm.After statistical data analysis,P=0.032,0.026 and 0.029,the difference was statistically significant(P<0.05).Polyp diameter in both groups decreased after treatment compared with before treatment.Independent t test was used for comparison between the two groups,and statistical analysis showed that P=0.021,0.013,0.011,the difference was statistically significant(P<0.05),indicating that the curative effect of the treatment group was generally better than that of the control group.3..Improvement of clinical symptoms:Before treatment,the average TCM syndrome score of the treatment group was 33.47;after 4,8 and 12 weeks of treatment,the average change of TCM syndrome score of the treatment group was 22.53,15.39 and 10.98,respectively;before treatment,the average score of the control group was 31.36;after 4,8 and 12 weeks of treatment,The mean change of syndrome integral was 25.91,19.70,16.77,and the data before and after the two groups were statistically analyzed,P<0.01,which showed significant difference,indicating that the TCM symptoms of the two groups were improved after treatment than before treatment.After treatment,independent t test and comparison were conducted between the treatment group and the control group.Statistical analysis showed that P=0.045,0.016,0.014,with significant differences(P<0.05),indicating that the effect of the treatment group on TCM symptoms was better than the control group.Conclusion:The prescription made with the addition and reduction of bupleurum Shugan powder can effectively reduce the diameter and number of gallbladder polyps,improve the clinical symptoms of patients with traditional Chinese medicine,and it is convenient to take clinically.So far,no obvious adverse reactions have been observed clinically,the basic biochemical indicators are normal before and after treatment,and the comprehensive clinical efficacy is better than that of the control group.It is proved that the addition and reduction of Chaihu Shugan Powder can effectively treat gallbladder polyps of liver-qi stagnation type.
