| Objective:This project aims to summarize and form a set of systematic theories for inheritance through the collection of clinical experience of Zhong’s anorectal school in treating perianal eczema.The randomized controlled trial was conducted to observe the clinical efficacy of Zhong’s Tufuling Kushen decoction combined with dermatitis cream in the treatment of damp-heat induced perianal eczema,preliminarily revealing the mechanism of the combined treatment,and providing basis for clinical application.Methods:1.Through daily outpatient consultation with doctors,interview with Professor Zhong Chuanhua,review of papers and works,medical records and other methods,the academic experience and thoughts of Professor Zhong Chuanhua on the differentiation and treatment of perianal eczema were collected,and summarized and sorted out.2.Sixty patients diagnosed as perianal eczema with damp-heat syndrome from December 2021 to December 2022 in the anorectal outpatient department of Kunming Hospital of Traditional Chinese Medicine were divided into two groups randomly.There were 30 cases in observation group and 30 cases in control group.The observation group was given Zhong’s Tufuling Kushen decoction cold and wet compress combined with dermatitis cream for external use,and the control group was given dermatitis cream for external use for 2 weeks.The scoring criteria adapted from Scorad(perianal pruritis,erythematous,papular/edema,exudation/scab,lichenification/rhagadia,range of skin lesions,and anal moisture)and Dermatology life quality index(DLQI)scores were recorded before and 2 weeks after treatment in the two groups.SPSS25.0 statistical software was used for correlation analysis,and the efficacy was evaluated by comparing the differences between the two groups after treatment.Results:1.Professor Zhong Chuanhua believed that the incidence of perianal eczema was caused by both internal and external factors,etiology and pathogenesis were based on dampness and combined with pathogenic factors,clear disease is based on syndrome differentiation of traditional Chinese medicine and staging of Western medicine,the therapeutic plan aims at curative effect and uses the combination of Chinese and Western medicines,conditioning care especially should attach importance to"three prevention"-prevent disease before it comes,prevent disease from changing,prevent recurrence after healing.Professor Zhong Chuanhua established the Tufuling Kushen decoction with the method of"clearing heat and dampness,dispelling wind and relieving itching".2.Among the 60 patients included in the study,during the study period,1 patient in the observation group did not take medication according to the plan,1 patient in the control group did not return to the clinic,2 cases were eliminated and 58 cases were completed.Treatment group and control group were 29 cases.(1)Before treatment,there were no significant differences between the observation group and the control group in gender,age,course of disease,Scorad score and DLQI score data(p>0.05),which were comparable.(2)After treatment,the total score of the observation group was 15.24±5.50,and the total score of the control group was 12.66±5.51.The total score of the two groups before and after treatment was analyzed by paired T-test,and there was a statistical difference(p<0.05),indicating that both groups were effective.(3)In the observation group,the total effective rate was 96.55%,2 cases were cured,15 cases were obviously effective,11 cases were effective and 1 case was ineffective;The total effective rate of the control group was 79.31%,including 1 case of recovery,6 cases of obvious effect,16 cases of effective and 6 cases of ineffective.,,and the data of the two groups were statistically different byχ~2test(p<0.05).The data of the two groups were statistically different byχ~2test analysis(p<0.05),indicating that the holistic therapeutic effect of the control group was inferior to the observation group.Conclusion:1.Professor Zhong Chuanhua is a representative inheritor of Zhong’s anorectal school with excellent medical skills.He treats perianal eczema in a variety of ways,flexibly combines Chinese and Western treatment methods,attaches importance to inducement and care,and develops proven prescriptions.Professor Zhong Chuanhua’s academic thoughts and experience are extensive and profound.Summarizing and sorting out his academic thoughts and experience is conducive to the academic inheritance of Zhong’s anorectal school and famous old Chinese medicine.2.The combination of cold and wet compress of Tufuling Kushen decoction and topical application of dermatitis cream can effectively relieve the clinical symptoms of damp-heat type perirectal eczema,repair the local diseased skin,and improve the quality of life of patients.The total effective rate of combined treatment is obviously better than that of Western medicine alone. |
