Objective:To observe the clinical efficacy of Trauma No.1 Ointment in the treatment of postoperative swelling and pain of distal radius fracture.Methods:Sixty-eight patients(20-80 years old)who were hospitalized in the four wards of the Bone Injury Center of Yunnan Provincial Hospital of Traditional Chinese Medicine from November 2021 to December 2022 due to fracture of the distal radius and received open reduction and internal fixation surgery, and had swelling and pain in the wrist after surgery, meeting the inclusion criteria.68 patients were divided into treatment group and control group by random number table method, with 34 patients in each group. The control group was given routine basic treatment plus functional exercise, and the treatment group was given Trauma No.1 Ointment on the basis of the control group. The VAS score, wrist circumference and swelling were recorded on the day before operation and the first day(the second day after operation),the fourth day(the fifth day after operation)and the seventh day(the eighth day after operation)after treatment. The wrist range of motion and wrist joint score were recorded and evaluated when the wrist X-ray examination was performed 2 months after the operation.Result:1.There was no significant difference between the two groups in general data(gender, age, fracture site),VAS score on the day before operation, circumference of the healthy side, circumference of the affected side and swelling degree(P>0.05).2.VAS score: On the 1th day after treatment, there was no statistically significant difference between the two groups in VAS score(P>0.05).On the fourth and seventh day after treatment, the VAS score of the treatment group was lower than that of the control group, with a statistically significant difference(P<0.05),indicating that on the fifth and eighth day after operation, the treatment group was better than the control group in reducing the pain of the affected limbs. The VAS score of the two groups after treatment was lower than that of the day before operation, and the VAS score of the 7th and 4th days after treatment was lower than that of the 4th and 1st days after treatment, with statistically significant difference(P<0.05),indicating that Trauma No.1 Ointment and functional exercise could gradually reduce the pain of the affected limb.3.Affected side circumferential diameter: on the 1st day after treatment, there was no statistically significant difference in the comparison between the two groups(P>0.05),and on the 4th and 7th days after treatment, the affected side circumference of the treatment group was lower than that of the control group, and the difference was statistically significant(P<0.05),indicating that on the 5th and 8th days after surgery, the treatment group was better than the control group in reducing the swelling of the affected limb. The intra-group comparison of swelling between the two groups after treatment was lower than that on the day before surgery, and the 4th and 7th days after treatment were compared with the 1st and 4th days after treatment, respectively, and the differences were statistically significant(P<0.05),indicating that both groups could effectively reduce the swelling of the affected limb.4.Swelling degree: On the 1st day after treatment, there was no significant difference in swelling between the two groups(P>0.05),and on the 4th and 7th days after treatment, the swelling degree of the treatment group was lower than that of the control group, and the difference was statistically significant(P<0.05),indicating that on the 5th and 8th days after surgery, the treatment group was better than the control group in reducing the swelling. The intra-group comparison of swelling between the two groups after treatment was lower than that on the day before surgery, and the difference between the 4th and 1st days after treatment and the 7th and 4th days after treatment were statistically significant, indicating that both groups could effectively reduce the swelling of the affected limb.5.Comparison of wrist range of motion: The difference in wrist range of motion between the two groups was statistically significant(P<0.05)after t-test at 2 months of treatment, indicating that after treatment, the treatment group was better than the control group in improving the range of motion of the affected limb.6.Wrist joint score and grade comparison: The wrist joint score and grade comparison between the two groups was tested by Mann-Whitney U at 2 months of treatment, and the difference was not statistically significant(P>0.05).7.Comparison of total clinical efficacy: After the efficacy of the two groups was tested by Mann-Whitney U,the total effective rate of the treatment group was 94.12%,and that of the control group was 85.29%,Z=-2.035,P=0.042<0.05,the difference was statistically significant, indicating that the total clinical efficacy of the treatment group was better than that of the control group. Conclusion:Both Trauma No.1 Ointment and functional exercise could improve the swelling, pain and wrist joint activity after the operation of distal radius fracture, and on the 4th and7 th days after the treatment,Trauma No.1 Ointment combined with functional exercise has a significantly better effect than functional exercise in improving the swelling and pain of the affected limb and joint activity. After 2 months of treatment, the total effective rate of the treatment group was better than that of the control group, and the improvement of wrist joint score and grade comparison was comparable between the two groups.
