Clinical Observation On The Regulation Of Immune Inflammation In Type 2 Diabetes Mellitus (Yin Deficiency And Dry Heat Type) By Modified Zishui Qinggan Decoction

ObjectiveTo observe the effect of modified Zishui Qinggan Decoction on immune inflammation regulation and clinical efficacy in patients with type 2 diabetes mellitus(Yin deficiency and dryness heat type).MethodsFrom November 2021 to November 2022,a total of 72 patients with type 2 di-abetes mellitus of yin deficiency and dryness heat type of diabetes were collected from the outpatient and inpatient departments of the Department of Geriatrics of the First People ’s Hospital of Yunnan Province.The patients who met the inclusion c-riteria were randomly divided into 36 cases in the control group and 36 cases in the treatment group.The control group was treated with standardized western medicine,and the treatment group was treated with modified Zishui Qinggan Decoction o-n the basis of standardized western medicine.SPSS22.0 was used for statistical analysis.The levels of high sensitivity C-reactive protein(hs-CRP),cytokine plasma int-erleukin 6(IL-6)and tumor necrosis factor α(TNF-α)were compared between the two groups before and after 2 weeks of treatment.The changes of fasting plasma gl-ucose(FPG),2-hour postprandial glucose(2h PG),glycosylated hemoglobin(Hb A1c)an-d TCM syndrome score were observed.To evaluate the effect of Zishui Qinggan Decoction on immune inflammation regulation and clinical efficacy in patients with type 2 diabetes(yin deficiency and dryness heat type).Results(1)A total of 67 patients were collected according to the plan,36 in the cont-rol group and 31 in the treatment group.No adverse events occurred during the st-udy.(2)There was no significant difference in HbA1c between the two groups befo-re and after treatment(P>0.05),indicating that the Hb A1 c level of the two groups did not improve significantly after treatment.(3)After treatment,FPG and 2h PG levels were lower than those before treatm-ent(P<0.01/P<0.05),and there was comparability between the groups(P<0.01/P<0.05),indicating that in reducing FPG and 2h PG levels,the treatment group had more advantages than the control group.Zishui Qinggan Decoction had a certain effect on the auxiliary hypoglycemic treatment.(4)After treatment,hs-CRP and IL-6 in the two groups were lower than befor-e(P<0.01/P<0.05),and the two groups were comparable(P<0.01).There was a differ-ence in TNF-α before and after treatment in the treatment group(P<0.05),but there was no significant difference in the control group(P>0.05).It is suggested that the levels of hs-CRP,TNF-α,IL-6 and other inflammatory factors in peripheral blood of patients decreased after the treatment of modified Zishui Qinggan Decoction,and the level of immune inflammation in type 2 diabetes decreased.It is further revealed that modified Zishui Qinggan Decoction has the effect of reducing immune i-nflammation in type 2 diabetes,and has good clinical application value.(5)After treatment,the effective rate of the two groups was 87.10 % in the tr-eatment group and 58.33 % in the control group.The treatment group was signific-antly higher than the control group(P<0.01).There was a significant difference in t-he total score of TCM syndromes before and after treatment in the treatment group(P<0.01),and there was a difference in the control group(P<0.05).The two groups were comparable(P<0.01).It shows that the curative effect of the treatment group is significantly higher than that of the control group.Zishui Qinggan Decoction combined with western medicine standardized treatment has more advantages in impro-ving the clinical symptoms of patients with type 2 diabetes(Yin deficiency and dry-ness heat type)than western medicine standardized treatment alone.ConclusionThe efficacy and safety of Zishui Qinggan Decoction combined with western medicine in the treatment of type 2 diabetes mellitus with yin deficiency and dryne-ss heat type are better.It can better improve the fasting blood glucose and 2-hour postprandial blood glucose levels of patients,and the levels of immune inflammato-ry factors(hs-CRP,cytokine IL-6,TNF-α)decrease,thereby reducing the immune in-flammation of type 2 diabetes mellitus and significantly improving the clinical symptoms of patients.
