Purpose:To carry out a study on the distribution pattern of TCM syndrome types of senile asthenia among inpatients in some hospitals of traditional Chinese medicine in Kunming area and the correlation between them and the coexistence of chronic diseases and multiple diseases,explore the syndrome differentiation standard and classification of the disease from the level of traditional medicine,provide clinical epidemiological basis for clarifying the evolution law of disease syndromes,and deepen the understanding of the risk factor of senile asthenia--the coexistence of multiple diseases.Improve early recognition of senile frailty.Methods:In this study,240 elderly patients(≥60 years old)who were hospitalized in geriatric wards of some hospitals of Traditional Chinese Medicine in Kunming from November 2021 to December 2022 were selected as research objects,and a cross-sectional study was carried out.Firstly,patients were divided into the frailty group and the non-frailty group with 120 cases each according to the evaluation results of Fried Frailty Scale.Data of four TCM diagnosis and related data of chronic disease comorbidities of included patients were collected,and the individual syndrome type was determined according to TCM syndrome diagnosis criteria.Then statistical software SPSS26.0 was used for statistical analysis of related data.Finally,the distribution law of TCM syndromes of senile asthenia and the correlation between senile asthenia and the coexistence of chronic diseases and multiple diseases were analyzed.Results:1.General data analysis: A total of 240 elderly patients participated in this study.(1)There was no significant difference in gender distribution between the two groups(P>0.05).There were 120 patients in the asthenic group,including 66 males(55%)and 54females(45%).There were 120 patients in the non-frailty group,including 55 males(45.8%)and 65 females(54.2%).(2)There was a statistically significant difference in age distribution between the two groups(P < 0.05).The most common age in the frailty group was 80 years old and above,and the most common age in the non-frailty group was 70-79 years old.The mean age of the frailty group(120 cases)was(79.71±7.05)years,and the mean age of the non-frailty group(120 cases)was(72.63±7.30)years.Among the age groups,there were 42 patients aged 60-69 years(7 patients in the asthenic group and 35 patients in the non-asthenic group),123 patients aged 70-79 years(53 patients in the asthenic group and 70 patients in the non-asthenic group),and 75 patients aged ≥80 years(60 patients in the asthenic group and 15 patients in the non-asthenic group).(3)The frailty scores of the frailty group in each age group were statistically significant(P< 0.05).The frailty score frequency of the frailty group was as follows: 3 points(12 cases),4 points(29 cases),5 points(31 cases),6 points(28 cases),7 points(20 cases);The frequency of fthenicity score in the non-fthenicity group was as follows: 0(40 cases),1(53cases),2(27 cases).2.Distribution of TCM syndrome types:There was no significant difference in the distribution of TCM syndrome types between the two groups.There was no significant difference in the distribution of TCM syndrome types of senile asthenia in gender.There was no significant difference in the distribution of different TCM syndromes in different age groups.The proportions of TCM syndrome types in the 240 cases were as follows in descending order: 77 cases(32.1%)of qi and blood deficiency,62 cases(25.8%)of spleen deficiency and phlegm-dampness,44 cases(18.3%)of spleen and kidney Yang deficiency,30 cases(12.5%)of five zang organs weakness,and 27 cases(11.3%)of kidney and essence deficiency.Qi and blood deficiency syndrome and spleen deficiency phlegm-dampness syndrome were the most common in both the asthenia group and the non-asthenia group.3.After sorting out the data of four TCM diagnosis of senile asthenia patients,the top 6symptoms of frequency distribution were respectively: fatigue in 105 cases(87.5%),dizziness in 101 cases(84.2%),waist and knee weakness in 98 cases(81.7%),insomnia in83 cases(69.2%),palpitation in 66 cases(55%),forgetfulness in 63 cases(52.5%);Tongue and pulse were most common in 100 cases(83.3%)and 93 cases(77.5%).The first 6symptoms in the non-asthenia group were: dizziness and dizziness in 103 cases(85.8%),fatigue in 101 cases(84.2%),insomnia in 100 cases(83.3%),palpitation and shortness of breath in 92 cases(76.7%),lumbago and knee weakness in 75 cases(62.5%),and disordered stool in 69 cases(57.5%).Tongue and pulse were most common in 88 cases(73.3%)and 90 cases(75%).4.All subjects in this study suffered from 8(2 ~ 14)chronic diseases on average,and the top three chronic diseases in terms of the number of comorbidity were hypertension in 164cases(68.3%),osteoarthropathy in 141 cases(58.8%)and diabetes in 107 cases(44.6%).At the same time,this study sorted out and analyzed the correlation between the number of debilitation and chronic disease comorbidity in elderly patients.Among the 240 cases included in the study,the number of chronic disease comorbidities in the senile fthenia group was successively: 29 cases of 1-5 diseases(24.2%),51 cases of 6-10 diseases(42.5%),and 40 cases of ≥11 diseases(33.3%).The number of chronic disease comorbidities in the non-fattening group was: 81 cases(67.5%)of 1-5 diseases,32 cases(26.7%)of 6-10 diseases,and 7 cases(5.8%)of ≥11 diseases.There was statistical difference in the distribution of the number of chronic disease comorbidities in the elderly patients(P < 0.05).Conclusion:1.Among the 240 cases included,the gender difference between the two groups was not statistically significant;The age difference was statistically significant,and senile frailty mostly occurred in people aged 80 years and above.The frailty score of the older group is higher,confirming that the frailty of old age is related to age.2.Among the 240 elderly inpatients with TCM syndrome,Qi and blood deficiency,spleen deficiency,phlegm-dampness syndrome were the most common,followed by spleen and kidney Yang deficiency,five viscera deficiency and kidney essence deficiency.There was no difference in the gender distribution of TCM syndrome between the senile asthenia group and the non-asthenia group.There was no significant difference in the distribution of TCM syndromes between the two groups and each age group.3.There were differences in the distribution of TCM syndrome between the two groups.The top 6 symptoms in the frequency distribution of the frailty group were fatigue,dizziness,waist and knee tenderness,insomnia,palpitation,shortness of breath,and forgetfulness.The patient’s tongue and pulse are most common with thin white tongue and thin pulse.The top 6 symptoms of frequency distribution in non-frailty group were dizziness,fatigue,insomnia,palpitation,shortness of breath,soreness and weakness of waist and knee,and irregular bowel movements.The patient’s tongue and pulse are most common with thin white tongue and thin pulse.4.The average number of chronic diseases in 240 elderly hospitalized patients was 8(2 ~14),and the top three chronic diseases in terms of the number of comorbidity were hypertension,osteoarthropathy and diabetes in descending order.This study also confirmed that there were differences in the distribution of the number of chronic disease comorbidities in elderly patients with frailty,that is,the more comorbidities,the more prone to frailty.
