Clinical Study On Juanbi Granule Combined With Small Needle Knife In Treating Ankylosing Spondylitis Of Cold And Dampness Obstruction Type

Purpose:1.The clinical efficacy and safety of Juanbi granules combined with small needle knife in the treatment of AS with cold and damp paralysis were investigated by analyzing the changes of IL-6,TNF-α,ESR,CRP and a series of other indexes.2.To provide a new method and medicine for the treatment of AS with the combination of internal and external Chinese medicine.Method:Patients who met the inclusion criteria for the study were grouped by random number expression and divided into the control group,treatment A group and treatment B group,with 30 cases in each group.All three groups were given oral Aceclofenac tablets,Juanbi granules in both treatment groups A and B,and small acupuncture knife in treatment group B.The total course of treatment was 28 days.After the observation and statistics were finished,Schober test,finger-ground distance,pillow-wall distance,VAS pain score,BASDAI score,BASFI score,ESR,CRP,IL-6,TNF-α,TCM syndrome score,TCM syndrome efficacy evaluation and ASDAS-CRP score were evaluated.the safety indicators were also evaluated before and after treatment,include blood routine,urine routine,stool routine+occult,liver function,kidney function,and electrocardiogram.Results:1.Comparison before and after treatment in each group:1.1 After analysis,ESR,CRP,IL-6,TNF-α,VAS score,BASDAI score,BASFI score,total score comparison of TCM syndrome,ASDAS-CRP score were all effective in the three groups before and after treatment(P<0.05).Among them,ESR,CRP,IL-6 and TNF-α indicators had significant curative effect before and after treatment(P<0.01).1.2 Although Schober test,finger-ground distance,pillow-wall distance were no difference before and after the treatment in the control group(P>0.05),but it had an improvement before and after treatment in Group A and Group B(P<0.05).2.Comparison among three groups after treatment:2.1 ESR,CRP,IL-6,TNF-α,VAS score,BASDAAI score,BASFI score,Schober test,finger-ground distance,pillow-wall distance,total score of TCM syndrome,TCM syndrome efficacy evaluation,ASDAS-CRP score treatment,the comparison among three groups indicates had a statistical difference(P<0.05).2.2 Pairwise comparison between groups within a group shows that:2.2.1 ESR,CRP,VAS score,BASFI score,pillow-wall distance,total score of TCM syndrome,TCM syndrome efficacy evaluation,ASDAS-CRP score: treatment group B >treatment group A > Control Group.The treatment effect of the group A and B were both better than that of the control group(P<0.05),and the group B was obviously better than that of the control group(P<0.01).Apart from this,group B was better than treatment group A(P<0.05).2.2.2 IL-6: Treatment Group B = Treatment Group A > Control Group.Treatment group A and treatment group B were more effective than the control group(P<0.05),but group B was equivalent to treatment group A(P>0.05).2.2.3 TNF-α: Treatment Group B > Treatment Group A > Control Group.Treatment group A and B were significantly better than the control group(P<0.01),and treatment group B was better than treatment group A(P<0.05).2.2.4 Schober test,finger-ground,BASDAI score: treatment group B > treatment group A > control group.Treatment group B had the best curative effect(P<0.01),and group A was also better than the control group(P<0.05).3.Safety indicators: No significant abnormalities were found in the three groups in the three major routine + occult blood,liver function,kidney function and other safety-related indicators,and there was no significant difference in the analysis of statistical results(P>0.05).Conclusion:1.The combination of Juanbi granules and small needle knife can effectively improve clinical symptoms and signs,reduce inflammatory indexes and relieve pain in AS patients,and its efficacy is better than NSAIDs alone or Juanbi granules combined with NSAIDs.2.Juanbi granules combined with small needle knife is significantly better than NSAIDs drug treatment alone in improving ESR,CRP,TNF-α,VAS score,BASFI score,Schober test,finger-ground distance,pillow-wall distance,total score of TCM syndrome,TCM syndrome efficacy evaluation,ASDAS-CRP score are significantly better than that of simple NSAIDs drug treatment.3.Juanbi granules can also improve the symptoms and signs of AS patients,reduce inflammatory indicators,and relieve pain.The clinical effect of Juanbi granules combined with NSAIDs is better than that of simple NSAIDs,and is also safe and effective.4.The combination of Juanbi granules and small needle knife is safe and effective,and is worthy of clinical use and promotion.
