ObjectiveIn this study,the syndrome element differentiation method was used to analyze the syndrome element characteristics of hypertension syndrome of phlegm,explore the correlation between hypertension syndrome of phlegm syndrome and CIMT,and study the risk factors of hypertension syndrome of phlegm syndrome,in order to provide reference for health education and behavior intervention of hypertension patients with phlegm syndrome.MethodsA total of 160 patients with hypertension admitted to the Department of Cardiovascular Medicine of the Affiliated People’s Hospital of Fujian University of Traditional Chinese Medicine from April 2022 to January 2023 were collected.The TCM health status identification system provided by the TCM syndrome Research Base of Fujian University of Chinese Medicine was used to identify the syndrome elements of patients,which were divided into phlegm syndrome group and non-phlegm syndrome group.Information,general information,body mass index(BMI),waist circumference(WC),total cholesterol,were collected from the four TCM examinations of the two groups.triglyceride(TG),high density lipoprotein cholesterol(high density lipoprotein cholesterol,HDL-C),low density lipoprotein cholesterol(LDL-C),apolipoprotein A1(Apo A1),apolipoprotein B(apolipoprotein B,Apo B),fasting blood glucose(FBG),carotid Intima-media thickness(CIMT),and a database was established.SPSS26.0 software was used for statistics,to explore the correlation between phlegm syndrome of hypertension and CIMT,and to study the syndrome element characteristics and risk factors of phlegm syndrome of hypertension.Results1.The BMI,WC,TG and LDL-C of the phlegm syndrome group were higher than those of the non-phlegm syndrome group(P < 0.05),and the number of people in the phlegm syndrome group with low exercise amount,moderate exercise amount,high salt diet,family history and drinking history was higher than that of the non-phlegm syndrome group(P <0.05).2.Binary logistic regression analysis showed that there was a history of alcohol consumption [OR = 11.79,95%CI(3.472-40.033),P < 0.001],overweight and obesity [OR =28.622,95%CI(8.485-96.547),P < 0.001],increased WC [OR = 1.110,95%CI(1.033-1.194),P = 0.005],increased LDL-C [OR = 1.110,95%CI(1.031-1.195),P = 0.006] were risk factors for phlegm syndrome of hypertension.Moderate exercise [OR = 0.123,95%CI(0.042-0.358),P< 0.001] was a protective factor for sputum syndrome of hypertension.3.Liver(78.64%),spleen(57.28%)and kidney(43.69%)were common syndromes in phlegm syndrome group.Blood stasis(77.67%),dampness(45.63%)and heat(32.04%)were the common syndromic elements with miscellaneous solids.The common syndromes of mixed deficiency were qi deficiency(61.17%),blood deficiency(32.04%)and Yang deficiency(32.04%).4.In the phlegm syndrome group of hypertension,CIMT thickening was more common(P <0.05).5.The difference of CIMT between grade 1 and Grade 3,grade 2 and grade 3 ofphlegm syndrome of hypertension was statistically significant(r < 0.05).The average rank of phlegm syndrome of Grade 1 was 81.90,Grade 2 was 74.33,and Grade 3 was 53.96.The smaller the average rank,the thicker the CIMT.6.In the correlation test between sputum syndrome pathological grade of hypertension and CIMT,G=0.494 in the symmetry measure(r < 0.001).In the linear trend test,the "linear correlation" χ~2 was 14.580(r < 0.001).Conclusions1.History of alcohol consumption,overweight and obesity,increased WC and elevated LDL-C were risk factors for hypertensive sputum syndrome,and moderate exercise was protective factor for hypertensive sputum syndrome.2.The common syndromes of phlegm syndrome of hypertension are liver,spleen and kidney.Common miscellaneous syndromes were blood stasis,dampness and heat,among which "blood stasis" was the main miscellaneous syndromes.Qi deficiency,blood deficiency and Yang deficiency were common syndromes with mixed deficiency,among which Qi deficiency was the main syndrome element with mixed deficiency.3.CIMT thickening is more common in patients with phlegm syndrome of hypertension.The CIMT of severe phlegm lesions was greater than that of mild and moderate lesions.The phlegm-syndrome score of hypertension was positively correlated with CIMT and showed a linear trend.With the increase of phlegm-syndrome score,CIMT had an increasing trend.
