Clinical Observation On The Therapeutic Effect Of Acupuncture On Hashimoto Thyroiditis Of Kidney Deficiency And Liver Depression Based On The Theory Of "Pei-Yuan Dispelling Depression"

Objective: To observe the clinical effect of acupuncture on Hashimoto’s thyroiditis with kidney deficiency and liver depression under the guidance of "Pei-yuan dispelling depression" theory.Methods: A total of 60 patients with HT in the Department of Endocrinology of Xiamen Hospital of Traditional Chinese Medicine(from January 2022 to December 2022)who met the diagnostic criteria of Chinese and Western medicine were randomly divided into treatment group(acupuncture group)by random number table method,including 30 males and 29females;Control group(Unimethylle group)included 30 cases,4 males and 26 females.The control group was treated with conventional eucalle(once a day for 12 weeks),and the treatment group,in addition to western medicine treatment,was treated with acupuncture at Taixi,Xuanzhong,Sanyinjiao,Taichong,Hegu,Neiguan,Fenglong,Renying,Futu and Guanyuan points on both limbs(once every other day for 12 weeks).The therapeutic effect was observed and evaluated after 12 weeks of treatment.Relevant data and evaluation data of FT3,FT4,TSH,TGAb,TPOAb titer before and after treatment,FT3,FT4,TSH titer 6 weeks after treatment,FT3,FT4,TSH,TGAb,TPOAb titer,TCM syndrome score,comprehensive efficacy evaluation and safety index were recorded.Collate the collected data and conduct statistical analysis through SPSS23.0.Results:1.After 1 course of treatment,in terms of TCM syndrome score and comprehensive efficacy evaluation,clinical symptoms of the treatment group were significantly improved after treatment,with statistical significance in intra-group comparison(P<0.05),while clinical symptoms of the control group were generally improved after treatment.Among them,the six syndroms-preneck gall,lazy speech of God,irritability,mood depression,waist and knee tenderness,and good Taiqi were significantly improved(P<0.05),but the three syndroms-god’s lazy speech,waist and knee tenderness,and decreased libido could not be improved(P>0.05).The treatment group was significantly better than the control group in improving symptoms(P<0.05).The total clinical effective rate was 90% in the treatment group and 73% in the control group.The total clinical effective rate in the treatment group was better than that in the control group(P<0.05).2.Thyroid autoantibody(TGAb,TPOAb)titers in both groups were significantly decreased after 12 weeks of treatment compared with before treatment(P<0.05),and the effect of reducing antibody titers in the treatment group was better than that in the control group after 12 weeks of treatment(P<0.05).3.In terms of thyroid hormone(FT3,FT4,TSH)titers,there were no significant differences in FT3 and FT4 titers between the two groups before treatment and 6 weeks after treatment(P>0.05),while there were significant differences in TSH titers between the two groups(P<0.05).There were statistically significant differences in FT3,FT4 and TSH titers between the two groups before treatment and 12 weeks after treatment(P<0.05).The improvement of thyroid hormone level in the treatment group was better than that in the control group after 12 weeks of treatment(P<0.05).Conclusions: Acupuncture can effectively relieve the clinical symptoms of HT patients under the guidance of the theory of "Peiyuan dispelling depression",and the therapeutic indexes of thyroid hormone(FT3,FT4,TSH),thyroid antibody(TGAb,TPOAb)titer,TCM syndrome score,comprehensive efficacy evaluation are improved,and the quality of life of patients is significantly improved.Therefore,acupuncture based on the theory of "Pei-yuan dispelling depression" has significant advantages in the treatment of HT,effectively delaying the progression of the disease,and has positive significance for the prognosis of the disease.
