| Objective:To observe the changes of depression degree,activities of daily living,serum IL-6 level and sertraline dosage before and after treatment in patients with post-stroke depression(PSD)by Pei Yuan Jie Yu acupuncture combined with sertraline,to provide new ideas and methods for acupuncture and moxibustion in the treatment of PSD.Methods:In this study,70 patients with PSD were randomly divided into the experimental group and the control group,35 cases in each group.The control group was given sertraline orally with the initial dose of 25mg·d(after breakfast)for 6 weeks;the experimental group was given Pei Yuan Jie Yu acupuncture on the basis of the control group,retaining the needle for30 min,once from Monday to Saturday morning,6 times/week,for 6 weeks.Hamilton Depression Scale-24(HDRS-24)was evaluated before treatment and after 2,4 and 6 weeks of treatment.The self rating Depression Scale(SDS)score,Barthel Index(BI)score,serum IL-6level and sertraline dosage before and 6 weeks after treatment were recorded.Results:1.Comparison of HDRS-24:there was a significant difference in the improvement score between the experimental group and the control group(P<0.05);there was a significant difference in the evaluation score at different times after treatment(P<0.05),which was the best after 6 weeks of treatment,followed by 4 weeks of treatment.There was interaction effect between time point and different treatment methods(P<0.05);there was no significant difference between the control group after 2 weeks of treatment and before treatment(P>0.05),but there was significant difference between the experimental group and the control group after 6 weeks of treatment(P<0.05).2.Comparison of SDS:after 6 weeks of treatment,the scores of the two groups were significantly lower than those before treatment,the difference was statistically significant(P<0.05);after 6 weeks of treatment,the scores of the experimental group were lower than those of the control group,the difference was statistically significant(P<0.05).3.Comparison of BI:after 6 weeks of treatment,the scores of the two groups were significantly higher than those before treatment,the difference was statistically significant(P<0.05);after 6 weeks of treatment,the scores of the experimental group were higher than those of the control group,the difference was statistically significant(P<0.05).4.Comparison of serum IL-6:after 6 weeks of treatment,the serum IL-6 levels of the two groups were significantly lower than those before treatment,the difference was statistically significant(P<0.05);after 6 weeks of treatment,the serum IL-6 level of the experimental group was lower than that of the control group,the difference was statistically significant(P<0.05).5.Comparison of sertraline dosage:after 6 weeks of treatment,the dosage of sertraline in the experimental group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusions:Pei Yuan Jie Yu acupuncture combined with sertraline in the treatment of PSD patients can effectively down regulate the expression of serum IL-6,improve the degree of depression and activities of daily living,reduce the dosage of sertraline,and promote the recovery of neurological function and improve the quality of life.The overall clinical effect is better than sertraline alone.The specific mechanism is speculated to be Pei Yuan Jie Yu acupuncture may down regulate serum IL-6.Therefore,acupuncture treatment of PSD is worthy of clinical promotion. |
