Observe The Effect Of Blood-letting By Collateral Puncture In The Treatment Of Tendinopathy Of Long Head Of Biceps Brachii Based On Musculoskeletal Ultrasound

Objective:Through musculoskeletal ultrasonography(MSUS)to scale,the depth of effusion within Long head of biceps tendon(LHBT)sheath and the thickness of LHBT,the effect and clinical effect of "Blood change pathogenic Elimination" collature-pricking and blood-letting therapy on tendinopathy of long head of biceps brachii which based on the guidance therapeutic philosophy of "Yi Tong Wei Yong".Methods:68 patients with tendinopathy of long head of biceps brachii who met the inclusion criteria were randomly divided into observation group("Blood-change pathogenic Elimination" collateral pricking and blood let therapy,twice a week,3 days apart)and control group(acupuncture group once a day,5 times a week),34 cases in each group.Four weeks was a course of treatment,and one courses of treatment were given to both groups.Before and after treatment,the measured values of MSUS(the depth of effusion within LHBT sheath,the thickness of LHBT)and shoulder joint score scale Constant-Murley(CMS)were collected and analyzed by SPSS25.0 software.Results:1.Baseline comparison: A total of 63 cases were included,31 cases in the control group and32 cases in the observation group.The two groups were consistent at baseline and the comparison conditions were satisfied.2.Result of the treatment:(1)The depth of effusion within LHBT sheath and the thickness of LHBT: the depth of effusion within LHBT sheath and the thickness of LHBT were significantly reduced after treatment in the observation group and the control group(P<0.05),and the difference was was statistically significant(P<0.05),the observation group was superior to the control group.(2)CMS: The total score of CMS in the two group after treatment was higher than that before(P<0.05),and the difference was statistically significant(P<0.05).The improvement of CMS score in the observation group was higher than that in the control group,and the difference between groups was statistically significant(P<0.05).Compared with the control group,the observation group had higher scores in pain degree,daily life and shoulder joint range of motion(P< 0.05).On the strength grade,the efficacy of the two groups was comparable(P>0.05).(3)The correlation between the depth of effusion within LHBT sheath and the score of CMS:The depth of effusion within LHBT sheath was negatively correlated with pain degree and shoulder joint activity,the correlation with daily life and muscle strength was excluded due to collinearity.(4)Evaluation of curative effect: 3 cases were cured,15 cases were markedly effective,12 cases were effective,2 case was ineffective,and the total effective rate was 93.75% in the observation group.0 cases were cured,11 cases were significantly effective,15 cases were effective,5 cases were ineffective,and the total effective rate was 83.87% in the control group.The curative effect of the observation group was better than that of the control group(P<0.05).Conclusions:1.The blood-letting therapy of "Blood-changing pathogenic Elimination" can reduce the depth of effusion within LHBT sheath and the thickness of the LHBT of patients with LHBB,and the effect is superior to conventional acupuncture.2.The blood-letting therapy of "Blood-changing pathogenic Elimination" can not only relieve the pain of shoulder joint,but also improve daily life and range of motion of shoulder joint in patients with LHBB,and the effect is superior to conventional acupuncture.3.With the increase of the depth of effusion within LHBT sheath,the pain scores and shoulder mobility decreased.
